Prophylactic antibiotics for adults with chronic obstructive pulmonary disease: a network meta-analysis

Abstract: BackgroundChronic obstructive pulmonary disease (COPD) is a chronic respiratory condition characterised by persistent respiratory symptoms and airflow limitation. Acute exacerbations punctuate the natural history of COPD and are associated with increased morbidity and mortality and disease progression. Chronic airflow limitation is caused by a combination of small airways (bronchitis) and parenchymal destruction (emphysema), which can impact day-to-day activities and overall quality of life. In carefully selec… Show more

“…Network meta-analyses have gained great importance in therapeutic research wherever a large number of CTs on different drugs are to be correlated. [5][6][7][8][9] This is currently particularly true in psoriasis, where a large number of approved drugs means that it is no longer possible to conduct complete clinical research with direct comparisons. Multiple comparisons are therefore used via bridge operators to create a comparable data situation.…”

NMAs have been published in different medical fields like cardiovascular diseases and diabetes, 5 pneumology 6 and psychiatry 7 . Also, interventions other than drug treatments were investigated, such as exercise in Alzheimer's disease 8 .…”

“…It is associated primarily with smoking, though additional factors are indoor smoke exposure, pollution, early-life respiratory infections, and genetics (87). Bacteria can be isolated by culture from sputum and BALF during exacerbations (88)(89)(90), and prophylactic macrolide antibiotics reduce exacerbations in advanced disease (91), implicating microbial drivers.…”

“…Non-cystic fibrosis bronchiectasis (nCFB) is a chronic pulmonary disease characterized pathologically by permanent dilation and chronic inflammation of the bronchi and bronchioles and clinically by chronic progressive cough and hemoptysis dyspnoea and exacerbated recurrent infection [ 1 ]. It is the final common pathway through which various inflammatory, hereditary, autoimmune, developmental, and allergic diseases funnel [ 2 , 3 , 4 , 5 , 6 ]. Compared to patients without nCFB, those with nCFB have higher all-cause-, respiratory- and lung cancer-related mortalities.…”
