Propensity Score-Matched Comparison of Partial to Whole-Gland Cryotherapy for Intermediate-Risk Prostate Cancer: An Analysis of the Cryo On-Line Data Registry Data

Abstract: Partial ablation results in better post-treatment sexual function compared with whole-gland ablation in men with intermediate-risk prostate cancer. We did not observe a difference in early BPFS between the two groups.

“…Procedural complication rates were generally low, with the most common being acute urinary retention (range 1.2-8.0%). When compared to whole gland cryotherapy, fCSAP resulted in a higher rate of erectile function preservation while continence and oncological outcome was similar for both options(19). Using mpMRI-guidance, fCSAP resulted in no deterioration in erectile function from baseline, and LUTS remained unchanged from baseline(20).…”

“…However, the present biopsy-proven 3 year 24.3% overall local recurrence rate is comparable to the 20.4% 1 year local recurrence reported by Barqawi et al following ultrasound-guided primary whole-gland CA 29 ; and broadly similar to other biopsy-proven estimates in similar studies. 30 For primary treatment, the 5 year local recurrence rate of 32.3% is slightly higher than estimates of biochemical recurrence at similar time-points in comparable ultrasound-guided CA series, 6,31,32 but significance is difficult to interpret due to small sample size and variable definitions. Notably, for patients with radio-recurrent PCa, 5 year local recurrence was only 25% in the present series.…”

“… 56 In 2017 Tay et al compared the results between 166 pairs of males with intermediate risk disease treated by ultrasound-guided focal or whole-gland CA and a significantly better sexual function at 12 months could be demonstrated for focal CA. 70 The limited follow-up period of this study (12 months) however, limits the conclusion on the safety profile of CA. In salvage treatments, the safety benefit of focal CA is even less evident.…”
