Focal therapy of prostate cancer is the targeted destruction of cancer within a specific part of the prostate gland, sparing the rest of the prostate and nearby tissue. This procedure could potentially reduce side effects when compared with established standard treatments, such as surgery or radiotherapy, which treat the entire prostate. Studies show that for most men with low-risk cancer, active surveillance is the preferred treatment option. However, the available data regarding all forms of focal therapy are still poor and inconclusive. Consequently, due to both the lack of clear results associated with focal therapy and the difficulties in detecting all cancerous areas of the prostate, focal therapy should be considered an investigational modality only.

Prostate cancer (PCa) is the most common malignant tumor in males, accounting for almost 1 in 5 new cancer diagnoses in the USA. 1 Despite recent reductions in age-standardized mortality, PCa remains the third most common cause of cancer-related mortality in males. 1,2 Standard curative treatment for prostate-confined disease includes radical prostatectomy and radiotherapy. 3 According to 2017 American Urological Association guidelines, whole-gland cryoablation (CA) is a third intention alternative curative therapy for patients unsuitable for standard treatments with good life expectancy (>10 years) and intermediate-risk disease (i.e. prostate specific antigen (PSA) 10.0-20.0 ng ml −1 , Gleason score = 7, cT2b-c); and may be offered to low-risk patients (PSA <10.0 ng ml −1 , Gleason Score <7, cT1c-T2a) following discussion of potential complications and absence of survival benefit compared with active surveillance. 4 CA has also been proposed as salvage therapy for patients with local recurrence following radical radiotherapy. 5 Several studies have established that whole-gland and focal CA for primary/radio-recurrent PCa provides acceptable

Prostate cancer (PCa) is the most common malignant tumor in males. The benefits in terms of overall reduction in specific mortality due to the widespread use of Prostate Specific Antigen (PSA) screening and the advancements in the curative treatments (radical prostatectomy or radiotherapy) appear to have reached a plateau. There remains, however, the questions of overdiagnosis and overtreatment of such patients. Currently, the main challenge in the treatment of patients with clinically organ-confined PCa is to offer an oncologically efficient treatment with as little morbidity as possible. Amongst the arising novel curative techniques for PCa, cryoablation (CA) is the most established one, which is also included in the NICE and AUA guidelines. CA is commonly performed under ultrasound guidance with the inherent limitations associated with this technique. The recent advancements in MRI have significantly improved the accuracy of detecting and characterizing a clinically significant PCa. This, alongside the development of wide bore interventional MR scanners, has opened the pathway for in bore PCa treatment. Under MRI guidance, PCa CA can be used either as a standard whole gland treatment or as a tumor targeted one. With MR-fluoroscopy, needle guidance capability, multiplanar and real-time visualization of the iceball, MRI eliminates the inherent limitations of ultrasound guidance and can potentially lead to a lower rate of local complications. The aim of this review article is to provide an overview about PCa CA with a more specific insight on MR guided PCa CA; the limitations, challenges and applications of this novel technique will be discussed.
