“…This online community serves as a guided practice model for increasing PCP self-efficacy and ability to manage complex conditions. 12,13 ECHO follows a hub-and-spoke model, connecting topicspecific specialty teams (the hub) with PCP sites (the spokes) to facilitate knowledge dissemination from academic health science centers to community-based providers.
