With the availability of a therapeutic target and an effective agent in trastuzumab, a systematic examination of the literature to investigate the role of human epidermal growth factor 2 (HER2) as a prognostic factor for survival and its association with clinicopathologic markers may improve treatment. An electronic search of the MEDLINE and PubMed databases (January 1990 to January 2011) was undertaken to identify translational studies that correlated HER2 with clinicopathologic markers and/or survival outcome. This review included 49 studies totaling 11,337 patients. Forty-four percent of patients had Stage I/II, and 56% had Stage III/IV disease. Immunohistochemistry was most commonly used to assess HER2 expression, identifying a median rate of 18% (range, 4-53%) of gastric cancer demonstrating HER2 overexpression. In patients with and without HER2 overexpression, the median 3-year disease-free survival rate was 58% (range, 50-88%) and 86% (range, 62-97%), respectively. Of the 35 studies reporting the impact of HER2 overexpression on survival, 20 studies (57%) reported no difference in overall survival, two studies (6%) reported significantly longer overall survival in patients with HER2 overexpression and 13 studies (37%) reported significantly poorer overall survival in patients with HER2 overexpression. The median overall survival and 5-year survival rate was 21 (range, 10-57) months and 42%, and 33 (range, 13-80) months and 52% in patients with and without HER2 overexpression, respectively. HER2 overexpression appears to be associated with poorer survival and with intestinal-type gastric cancer in this group of patients for whom majority undergone curative gastrectomy.On a global scale, gastric cancer remains the second most common cause of cancer-related death with an estimated 930,000 new cases diagnosed each year albeit an overall decline in incidence. 1 Noncardia gastric cancers have steadily declined. However, it has been observed that proximal gastric cancers (cardia and oesophago-gastric tumors) are increasing at a rate of up to 10% per year, far exceeding that of any other type of cancer. Metastatic or advanced unresectable gastric cancer is an aggressive malignancy with a poor prognosis despite chemotherapy treatment with median survival of 7.5 to 12 months and a median survival of 3 to 5 months if managed by best supportive care. 2 Gastric cancer is a chemosensitive disease where a range of chemotherapy agents may achieve response rates of 30 to 40%; however, in the advanced setting, the disease is limited by a low complete response rate and the response duration is protracted often leading to disease progression and treatment related toxicities. 3 Combination chemotherapy with a Japanese standard doublet of fluoropyrimidine and platinum or western triplet regimens has shown better response rates and outcomes over single agent chemotherapy. Abstract:The c-erbB-2 (HER-2/neu), EGF and EGFR (erbB-1) proteins, members of the epidermal growth factor receptor family, play a role in cell growth by binding to cell membrane receptors. The aim of the current study was to evaluate the expression of c-erbB-2, EGF and EGFR in advanced gastric carcinoma and to analyze its relationship with chosen anatomo-clinical parameters and prognosis. Standard avidin-biotin-peroxidase was used for c-erbB-2, EGF and EGFR immunohistochemical staining (Novostain Super ABC Kit Universal); anti-human c-erbB-2 protein monoclonal antibody NCL-cerbB-2-316, anti-Epidermal Growth Factor monoclonal antibody (clone EGF-10) and EGFR goat polyclonal IgG (p-EGFR). A statistically significant correlation was found between c-erbB-2, EGF, EGRF expressions in the main mass of tumor and lymph node metastasis (p=0.000; p=0.000; p=0.00001, respectively). Also an association was observed between c-erbB-2 expression and Bormann's and Lauren's classifications (p=0.05; p=0.006, respectively). Similarly, the expression of EGFR in main mass of tumor was correlated with the depth of invasion (p=0.007) and histological differentiation (p=0.04). Moreover, the expression of c-erbB-2 in the main mass of tumor and lymph node metastasis was associated with the age of the patients (p=0.03; p=0.0002 respectively). Strong association was found between the expression of EGRF in lymph node metastasis and histological differentiation (p=0.04). Positive expression of c-erbB-2 in lymph node metastasis was correlated with lymph node involvement (p=0.04). Positive expression of c-erbB-2 in the main mass of tumor and in lymph node metastasis was strongly correlated with postoperative survival (p=0.00001; p=0.003 respectively). We also found a relationship between EGF expression in gastric tumor and survival time (p=0.003). No association was noted between the expression of EGFR in the main mass of tumor and in lymph node metastasis and between the expression of EGF in lymph node metastasis and survival time. Our results suggest that the expression of c-erbB-2 and EGF protein can help predict the postoperative survival time.

82Рак желудка продолжает оставаться одним из самых распро-страненных злокачественных заболеваний в мире [1, 2]. В Рос-сийской Федерации в 2013 г. в структуре онкологической заболе-ваемости эта патология заняла 4-е место (8,8%) у мужчин и 5-е (5,7%) у женщин [3]. Несмотря на определенную тенденцию к снижению заболеваемости раком желудка в большинстве стран мира [1, 3], результаты лечения этой патологии нельзя считать удовлетворительными. В структуре смертности от злокачествен-ных новообразований рак желудка прочно занимает 2-е место [3, 4]. Это связано с тем, что более 70% больных впервые обращают-ся за медицинской помощью, уже имея III или IV стадию заболе-вания. Как следствие, летальность от рака желудка в течение первого года после установления диагноза составляет около 50% [5], а 5-летняя выживаемость радикально оперированных боль-ных в большинстве стран мира не превышает 15-30% [2, 4]. В связи с этим определение факторов, способствующих прогрес-сии рака желудка, чрезвычайно важно как для прогнозирования течения заболевания, планирования адекватной терапии, так и для поиска новых направлений в лечении этой патологии.Целью настоящего обзора явилось освещение важнейших клинико-морфологических и молекулярно-биологических фак-торов, определяющих прогноз и течение рака желудка.Согласно данным японского общества по изучению рака, стадия заболевания, гистологическая структура опухоли, лимфа-тическая и венозная инвазия являются ведущими клинико-мор-фологическими факторами прогноза рака желудка [6, 7]. Наибо-лее низкие показатели выживаемости отмечены при глубине ин-вазии Т3 и Т4; при наличии метастазов в регионарных лимфати-ческих узлах, особенно при N2 и N3; при наличии лимфатиче-ской и венозной инвазии; при низкодифференцированной и му-цинозной аденокарциноме, недифференцированном и желези-сто-плоскоклеточном раке; диффузно-инфильтративном и ин-фильтративно-язвенном раке желудка [6, 7].Следует отметить, что при отсутствии отдаленных метаста-зов решающее значение для прогноза имеет поражение регио-нарных лимфатических узлов. Значение имеет не только наличие метастазов, но и количество пораженных лимфатических узлов, их локализация [8][9][10][11]. Риск лимфогенного метастазирования повышается с увеличением размера и глубины инвазии опухоли [8, 9, 12, 13] при диффузном типе рака желудка, низкодифферен-цированных и недифференцированных опухолях [14,15], при на-личии лимфатической и венозной инвазии [8, 9, 13,16].Ухудшение отдаленных результатов лечения отмечено при локализации опухоли в проксимальной трети желудка [7,17,18] и у лиц молодого возраста [19,20].
