“…A cirurgia parendodôntica apresenta uma alta taxa de sucesso de 95,3%, a contribuição para o resultado está no emprego do microscópio operatório, facilitando a visão do campo operatório, além de técnicas modernas, utilização do ultrassom com pontas Brazilian Journal of Health Review, Curitiba, v. 6, n. 1, p. 872-885, jan./feb., 2023 anguladas, instrumentos microcirúrgicos e novos materiais retrobturadores. (PALLARÉS-SERRANO et al, 2021;SUKEGAWA et al, 2022) A avaliação pré-operatória é uma das etapas fundamentais para um correto planejamento cirúrgico e ela engloba histórico médico e odontológico, exames intra-orais e extra-orais e exames complementares incluindo principalmente o uso de exames de imagens como radiografias tridimensionais (3-D) como a TCFC para um correto diagnóstico e para avaliação das estruturas anatômicas como: espessura óssea, tamanho da lesão e a correta posição de raízes como a disto lingual. A proximidade do elemento com o seio maxilar não invalida a cirurgia, porém, é necessário cautela para que não ocorra comunicação ou a entrada de resíduos infectados para dentro desse seio, na mandíbula também é necessário ter cuidado devido a presença do forame mentual e do nervo alveolar inferior (CHONG e RHODES, 2014;WANG et al, 2017) Na cirurgia endodôntica moderna a osteotomia pode variar de 4-5 mm, tamanho suficiente para fornecer espaço para a ponta ultrassônica de 3 mm, tendo que levar em conta também que na maior parte dos casos de cirurgia paredodôntica se tem a presença de uma lesão e consequentemente uma destruição da cortical óssea na região apical, o que facilita a localização e determina se será necessário ou não a realização da osteotomia que fica de acordo com essa destruição ao redor do ápice (KIM e KIM, 2019).…”
