Prognostic Factors Affecting the Clinical Outcome of Adenoid Cystic Carcinoma of the Head and Neck

Abstract: High tumor grade and lymph node involvement were predictive of recurrence and overall survival, respectively. Despite aggressive treatment, it seems to be impossible to prevent the development of distant metastasis. Therefore, more research is needed to identify molecular biomarkers that predict the clinical outcome and to develop effective treatment for patients with ACC.

“…However, the long-term outcome of these treatments is unfavorable (6). Several studies have identified clinicopathological factors in ACC with an unfavorable effect on survival, including old age, tumor location, advanced stage, solid histological subtype, high grade, tumor-node-metastasis (TNM) staging, major nerve involvement, the presence of perineural invasion, a positive surgical margin and lymph node metastasis (7,8). However, the real prognostic and recurrence factors are still unknown.…”

“…Consequently, the prognosis of adenoid cystic carcinoma remains clinically unpredictable. 6,7 Furthermore, considerable previous research has linked Ki-67 expression with the aggressiveness and prognosis of adenoid cystic carcinoma, although the results are still perplexing. 8 For these reasons, the research reported here used previously unstudied samples of adenoid cystic carcinoma supplied by the Department of Anatomical Pathology, Dr. Hasan Sadikin Hospital, Bandung.…”

“…6 In other words, the prognosis of adenoid cystic carcinoma is clinically unpredictable. 7 Furthermore, a considerable body of research has linked Ki-67 expression with the aggressiveness and prognosis of adenoid cystic carcinoma, although results remain inconclusive. 8 Consequently, it is important to predict the prognosis of the tumor.…”

“…The common sites of metastasis are lungs, bone, liver and brain. Metastasis to the spinal vertebrae is extremely rare though perineural invasion in margin positive surgical resections have been noted in literature [3][4][5][6]. Typically, low back or neck pain is the commonest presentation of spinal metatstasis, followed by weakness, sensory loss, and bladder dysfunction [7].…”

“…The median time to locoregional recurrence is two years and distant metastasis is three years with a three and five year disease free survival rates being 64.3% and 36.1% respectively [6].…”
