In order to estimate the potential risk of chemicals including drug in patients with type 2 diabetes mellitus (T2DM), we investigated allyl alcohol induced liver injury using SD rats and Spontaneously Diabetic Torii-Lepr fa (SDT fatty) rats as a model for human T2DM. The diabetic state is one of the risk factors for chemically induced liver injury because of lower levels of glutathione for detoxification by conjugation with chemicals and environmental pollutants and their reactive metabolites. Allyl alcohol is metabolized to a highly reactive unsaturated aldehyde, acrolein, which is detoxified by conjugation with glutathione. Therefore, we used allyl alcohol as a model compound. Our investigations showed that SDT fatty rats appropriately mimic the diabetic state in humans. The profiles of glucose metabolism, hepatic function tests and glutathione synthesis in the SDT fatty rats were similar to those in patients with T2DM. Five-week oral dosing with allyl alcohol to the SDT fatty rats revealed that the allyl alcohol induced liver injury was markedly enhanced in the SDT fatty rats when compared with the SD rats and the difference was considered to be due to lower hepatic detoxification of acrolein, the reactive metabolite of allyl alcohol, by depleted hepatic glutathione synthesis. Taking all the results of the present study into consideration, the potential for allyl alcohol to induce liver injury is considered to be higher in diabetic patients than in healthy humans.

Objective: To study the biochemical pattern of liver function tests in adult patients of diabetes and the association with hypoglycemic medications. Study Design: Cross-sectional study. Place and Duration of Study: Creek General Hospital, Karachi Pakistan, from Jun 2021 to Sep 2021. Methodology: Two sixty patients were enrolled through consecutive non-probability sampling. Adult patients with diagnosed Type 2 diabetes mellitus were included in the study. While Type 1 diabetes patients, pregnant subjects, and people with diabetes with any other systemic disease or on any other non-diabetic medications or any history of drug abuse were excluded from the study. Results: Raised Alanine transaminase levels were seen in 193 (74%) patients, a high Alkaline Phosphatase in 208 (80%) and a high Aspartate Aminotransferase in 202 (77%) patients. Liver markers were higher in the middle age group, females, hypertensive, and with abdominal obesity. There was no association between glycemic control and the duration of diabetes. The number of subjects having raised ALT and ALP was higher in those taking oral hypoglycemic and lower in those with insulin. Conclusion: The liver markers are raised in patients with diabetes and can be the predictors of future hepatotoxicity.

ABSTRACT:Introduction: Liver is a vital organ and mediates many cellular processes. Liver may be injured in diabetes. Metformin treatment shows a high potential to lower the risk of liver injury or to ameliorate its damage. Study objective:To explore the potential of metformin treatment in reducing the level of liver injury. Methods and subjects:The present study employed the retrospective study design to review the files of diabetic patients. Study variables includedgender, age, duration of diabetes, metformin treatment, and liver function tests. Datawere analyzed using SPSS version 21. The relationship between variables was examined using Chi-square test, and T test. Significance was considered if p< 0.05. Study results:The study included 62 diabetic patients. The mean age of study participants was 62.95± 11.98 years, a total of 33 (53.22%) of study participants were males, duration of diabetes was 7.51±5.86 years, metformin was used as a diabetic therapeutic option by 41 (66.13%), the mean dose of metformin was 1617.32±694.49 mg, the mean level of AST was 16.01±7.79 (U/L), the mean level of ALT was 18.500±8.89 (U/L), and the mean level of bilirubin was 0.38±0.18 (mg/dl). A significant relationship was found between metformin dose and each of liver enzymes (p=0.000). When the mean levels of liver enzymes were compared between metformin group and without metformin group, no significant differences were observed. Conclusion:Liver injury in diabetes is a long term process and the use of metformin can offer protection against liver injury through keeping liver enzymes within normal limits.
