Introduction: Space closure is one of the most challenging processes in Orthodontics and requires a solid comprehension of biomechanics in order to avoid undesirable side effects. Understanding the biomechanical basis of space closure better enables clinicians to determine anchorage and treatment options. In spite of the variety of appliance designs, space closure can be performed by means of friction or frictionless mechanics, and each technique has its advantages and disadvantages. Friction mechanics or sliding mechanics is attractive because of its simplicity; the space site is closed by means of elastics or coil springs to provide force, and the brackets slide on the orthodontic archwire. On the other hand, frictionless mechanics uses loop bends to generate force to close the space site, allowing differential moments in the active and reactive units, leading to a less or more anchorage control, depending on the situation. Objective: This article will discuss various theoretical aspects and methods of space closure based on biomechanical concepts.

Summary Introduction In everyday clinical practice, we often encounter a lack of space for placing all the teeth present into dental arch; therefore it is often recommended to extract teeth within orthodontic treatment. In clinical practice, the most commonly used methods of closing space after are elastic chains and NiTi closed spiral springs. The aim of this paper was to compare postextraction space closure speed using two different sliding mechanisms, NiTi closed coil springs and elastic chains within treatment with fixed orthodontic appliances. Material and Method The total sample in this study consisted of 46 postexstraction spaces in 23 patients indicated for the extraction of first premolars using treatment with fixed orthodontic appliances. Two sliding mechanisms, NiTi closed coil spring and elastic chains were applied to postextraction space closure. Postextraction spaces were monitored for 4 months with appointments every 4 weeks. During appointments mechanisms were activated and digital caliper was used to measure the width of the postextraction space. Results The results showed that NiTi closed coil springs method achieved greater reduction in postexstraction space (3.94 mm) while with elastic chain method the closure of 3.10 mm was achieved. The total difference between these two methods in the observed period was 0.84mm and no statistically significant difference was found (p> 0.05). The lowest value for NiTi closed springs was 2.19 mm, while for the elastic chain it was 1.29 mm. The best postextraction space closure was 5.70 mm and it was completed using NiTi closed springs while for elastic chain the best value was 4.80 mm. Conclusion: NiTi closed coil springs lead to faster closure of postextraction spaces in relation to elastic chain. Since this difference is minimal, in practice, both methods can be used equally.

Summary Background/Objectives The aim of this systematic review was to compare the different force delivery systems for orthodontic space closure by sliding mechanics. Search methods Multiple sources including Cochrane Central, Ovid Medline, Embase etc. were used to identify all relevant studies. Selection criteria Randomized controlled trials (RCT) of parallel-group and split-mouth designs were included. Participants Orthodontic patients of any age treated with fixed orthodontic appliances and requiring space closure. Data collection and analysis Search result screening, data extraction and quality assessment were performed independently and in duplicate by 2 reviewers. The included studies were grouped into parallel-arm and split-mouth studies and subgroup analysis was then performed for the type of retraction subsets; en-masse and individual canine retraction. A traditional meta-analysis, and network meta-analysis (NMA) for direct and indirect comparisons for the rate of space closure were performed. Results Thirteen studies, six parallel-arm and seven split-mouth were included. The traditional meta-analysis comparing Nickel-titanium (NiTi) closed coil springs and elastomeric power chain for the rate of tooth movement showed statistically significant difference favouring NiTi springs (MD: 0.24; 95% CI, 0.03–0.45; I2 0%, P = 0.02) and the comparison between NiTi springs and active ligatures also showed statistically significant result favouring NiTi springs (MD: 0.53; 95% CI, 0.44–0.63; I2 0%, P ˂ 0.00001) for the rate of tooth movement. NMA for the rate of space closure showed fairly confident evidence for NiTi coil springs when compared with elastomeric chain and active ligatures. The NiTi coil spring ranked best between all methods of space closure. Conclusions There is moderate quality evidence in favour of NiTi coil springs for the rate of space closure when compared with active ligature and low quality of evidence favouring NiTi springs when compared with elastomeric chain. The ranking from NMA showed NiTi coil springs to be the best method for space closure with 99% chance. An urgent need for standardization of study designs and the need for development of an agreed core outcome sets and core outcome instrument measurement sets is evident.
