Procalcitoninplasmakonzentration und systemische inflammatorische Antwort nach verschiedenen operativen Eingriffen

Abstract: Postoperative PCT plasma concentrations in patients presenting without signs of infection are largely influenced by the type of surgical procedure. During the first and second postoperative day PCT concentrations are more frequently elevated in patients after major abdominal, major vascular and thoracic surgery compared to patients undergoing minor, aseptic operations. Thus an "infection monitoring" considering PCT value analysis during the postoperative course may transiently be impeded after major and partic… Show more

“…Other studies have confirmed this suggestion [1, 16, 23, 28, 53, 54]. The results of the present meta-analysis confirm these results demonstrating that average values of CRP differ between patients with and without infectious complications.…”

“…Several studies have concluded that CRP is a useful predictor of PIC, but low positive predictive values have been reported [7, 1623], making CRP a suboptimal marker for ruling in of an infectious complication. A recent meta-analysis of CRP after gastroesophageal cancer surgery confirms that CRP values are insufficient to predict postoperative inflammatory conditions [24].…”

“…Valuable information about the postoperative behaviour of PCT derives from three studies, designed to assess [76] reference values after different types of surgery by investigating patients without any kind of complication at all [58,78,79]. Depending on the type and extent of surgery, the authors found a considerable rise in PCT levels within the first postoperative week.…”

“…Molter et al [26 ]were able to show that the determination of procalcitonin is less sensitive than the determination of the CRP value. As in our study, they came to the conclusion that CRP is only of limited value for the differentiation between inflammation and postoperative tissue trauma.…”
