Procalcitonin for guidance of antibiotic therapy

Abstract: Procalcitonin is a surrogate biomarker for estimating the likelihood of a bacterial infection. Procalcitonin-guided initiation and termination of antibiotic therapy is a novel approach utilized to reduce antibiotic overuse. This is essential to decrease the risk of side effects and emerging bacterial multiresistance. Interpretation of procalcitonin levels must always comprise the clinical setting and knowledge about assay characteristics. Only highly sensitive procalcitonin assays should be used in clinical pr… Show more

“…First, the use of this analyte in conjunction with a limited number of demographic and clinical, and possibly other laboratory, variables may be a practical and effective approach. This approach conforms to the widely accepted adage that biomarker values should be interpreted within the comprehensive context of each particular case rather than in isolation [91].…”

“…The use of PCT supplies caregivers with added information, which, in principle, enables them to improve the selection of patients for treatment, the timing of treatment initiation, and the overall duration of treatment [21,28,32,33,35,[41][42][43][44][45][46][47]. Insofar as caregivers change their care management and treatment strategies in response to the new information (relative to the usual standard course of action), there are implications for changes in outcomes, both in terms of treatment costs and health status [14,33,35].…”

“…Serum PCT based algorithm has been designed and used to guide the management of sepsis in many countries in Europe and the USA [7][8][9][10]. Several formats are available for the determination of serum PCT, 1) a semi quantitative lateral flow rapid test device called PCT-Q (> 0.5 ng·ml -1 ); 2) a fairly sensitive (>0.2 ng·ml -1…”
