Primary pure non-gestational choriocarcinoma of ovary: a rare case report

Abstract: Germ cell malignancies represent 15% of ovarian cancers. Choriocarcinoma is a malignant tumour with trophoblastic differentiation. It may be gestational or non-gestational and may be a primary tumour or metastatic from other organs. Pure non-gestational choriocarcinoma is an extremely rare neoplasm, with reports of only a few cases. This study reports a rare case of non-gestational pure choriocarcinoma in a postmenarcheal young female and describes details of the tumour, including the clinicopathological findi… Show more

“…16 A case report of a young women with a clinical suspicion of ruptured ectopic pregnancy underwent emergency laparotomy with an intra op finding of ovarian mass with bleeding later diagnosed as primary pure non-gestational choriocarcinoma of ovary. 17 The only case report of a 13-year-old girl with pure choriocarcinoma in turkey managed with triple chemotherapy consisting of metho-trexate, actinomycin and cyclophosphamide that was started 2 weeks following the surgery at an oncology center and was followed with β-hCG and the patient received one additional course of chemo-therapy. 18…”

“…At beta-HCG levels seen in NGOC cases, the Hook Effect becomes relevant [32]. When using point of care urine pregnancy tests, the Hook Effect occurs in sandwich immunoassays when the antigen concentration is high enough to saturate both the migratory phase and fixed detection antibodies independently, rather than binding occurring to subunits of the same molecule [14,15]. Thus, a falsely negative test could potentially lead to a delay in diagnosis and allow for progression of disease [33].…”

“…There are two types of NGOC: the pure type and the mixed type. The pure type, which is extremely rare, only contains choriocarcinoma [14]; the absence of other germ cell elements is demonstrated by the lack of immunohistochemical staining for CD30, PLAP, and AFP [15]. The mixed type contains other germ cell tumors, including immature teratomas, endodermal sinus tumors, embryonal carcinomas, and dysgerminomas [13].…”

“… 4 Generally, NGCO have been clinically diagnosed in female patients who were sexually immature, unable to conceive, or have never had sexual intercourse. 5 Ovarian germ cell tumours commonly occur in the first two decades of life, but can also be seen in any age group. 6…”
