Primary Protrusion of the Acetabulum

Abstract: 1. A study of fifty-nine patients with protrusion of the acetabulum showed that they fell into three age groups: those in their teens, those aged thirty-five to fifty years, and those presenting from fifty-one years onwards. 2. The juvenile age group was investigated clinically and biochemically but no causative metabolic factor was found. 3. The familial incidence was confirmed by radiological and biochemical surveys of several families. 4. The results of treatment show that when conservative treatment is … Show more

“…It can even lead to an amplification of a potential conflict between the posteroinferior femoral head-neck junction and the posterior horn of the acetabulum [17,18]. This might explain the variability of reported results ranging from promising midterm results [21] to sobering results after short-term followup [12]. There is only one previous report [17] of open acetabular rim trimming and two case reports [20,30] of a total of six cases with arthroscopic circumferential rim trimming.…”

“…1) [17,44]. The acetabulum usually presents with an excessively increased size of the lunate surface [40], which in turn leads to excessive femoral coverage [44] with a lateral center-edge angle typically exceeding 39° [12,47]. Protrusio acetabuli has been described as the most severe representation of pincer-type femoroacetabular impingement (FAI) [18].…”

“…Various radiological methods have been used to evaluate this complication in adults [2,10,13,19]. To our knowledge no systematic evaluation of the normal bony landmarks has been undertaken in children in order to detect protrusio in its earliest stages.…”

“…jection of a bony bulge inside the pelvic cavity [13], the presence ofa Wiberg's centre edge (CE) angle greater than 45 ~ [19], the presence of crossed and reversed teardrop [10], and recently the crossing of the acetabular line medial to the ilioischial line by 3 mm or more in men and by 6 mm or more in women [2]. This last method has proved to be more reliable in measurement of protrusio than previous methods.…”

“…A protrusão acetabular foi inicialmente descrita em 1824 por Adolph William Otto, patologista alemão, por meio de estudos realizados em cadáveres, como uma deformidade da parede medial do acetábulo, com consequente migração da cabeça femoral para o interior da pelve [1][2][3][4][5][6] .…”
