Primary Osteogenic Sarcoma with Pulmonary Metastasis: Clinical Results and Prognostic Factors in 91 Patients

Abstract: The prognosis of osteosarcoma with pulmonary metastases is dismal, especially for patients who have primary pulmonary metastases, more than three pulmonary metastatic nodules or involvement of more than one lobe. Factors such as older age, larger tumor volume and elevated LDH may reflect high metastatic rate.

“…It mainly occurs in children and adolescents and is characterized by local aggressiveness and high rates of lung metastasis (1). It is reported that approximately 20% of osteosarcoma patients exhibit lung metastasis at diagnosis, and for the rest, 2 out of 5 patients were diagnosed with metastatic lesions at a later stage (2). With the refinement of multimodality treatments, including neoadjuvant chemotherapy, surgical techniques and radiotherapy, in the most recent two decades, the five-year BMPR2 contains three domains: a short extracellular binding one, a transmembrane one and an intracellular one with serine/threonine kinase activity (5).…”

“…The relatively high mortality of osteosarcoma is predominantly associated with systemic metastasis, especially the pulmonary metastasis (14). Despite aggressive treatment modalities, such as high-dose chemotherapy and wide tumor resection, the patient's 5-year survival rate is only 55%-60%, and even less than 40% with pulmonary metastasis (15). Meanwhile, high-dose chemotherapy has lots of adverse reactions such as gastrointestinal reactions, bone marrow suppression, cardiac toxicity, renal toxicity, which restrict the application of chemotherapy.…”
