Primary Mucosal and Glans Penis Melanomas: The Sydney Melanoma Unit Experience

Abstract: Possible reasons for poor survival include: (i) non-specific symptoms resulting in late presentation; (ii) locally advanced disease not being recognized by a clinician as a rare form of melanoma, resulting in a delay in treatment; (iii) anatomical constraints precluding surgery with generous margins and consequently resulting in a high incidence of local recurrence. Also, rich vascularity and multiple lymphatic drainage pathways may mean a predisposition to early dissemination. Prompt diagnosis and referral to… Show more

“…MM arising in the genital area is rare, we had only a few cases, but our results confirm the findings in other population that show a higher frequency of MM in the genital area in females than males [15, 20]. On the other hand, oncogenic human papillomavirus (HPV) types such as HPV 16, has been detected in two cases of vulvar MM.…”

“…This may be attributed to the advanced disease at the time of initial diagnosis, rich vascular and lymphatic supply of mucosal sites, and the lack of clinical suspicion of these tumors because of their rarities. The prognostic factors include age, gender, anatomic site, clinical stage, and ulceration of the primary lesions (32,33).The treatment modalities rely mainly on surgical intervention, with little use of chemotherapy. The five-year survivals are below 50% in most series (34).…”

“…Primary melanoma of the male urethra is an uncommon tumour, with fewer than 50 reported cases [1]. Overall the prognosis is poor, with conventional prognostic markers used in cutaneous melanomas being less useful at predicting survival [1,2]. Surgical excision with adequate margins is the mainstay of managing melanoma.…”
