Primary mucocele of the mastoid: An incidental finding

Abstract: Mucocele is an accumulation of secretion products, desquamation, and inflammation within a body cavity: Localization in the mastoid is extremely rare. Erosion of bony walls and invasion of surrounding structures expose a patient to intra‐ and extracranial complications. Proper imaging work‐up and complete removal through mastoidectomy is warranted.

“…The accepted pathogenic basis of mucocele is obstruction of a natural drainage pathway, although gradually enlarging mucus retention cyst is also suggested as an alternative hypothesis in temporal bone. Mucoceles can develop as a primary condition or as a secondary condition resulting from persistent inflammation, trauma, scarring from prior surgery or radiotherapy, or, less frequently, neoplasms 1 3 . We present a rare case of 60-year-old male with temporal-mastoid mucocele with hypertrophy of the temporal bone.…”

“…As previously alluded to, when performing mSTP, the authors remove enough bone to fully visualize and remove disease, but do not usually perform extensive bony exenteration of uninvolved bone as originally described in the literature (12). In contrast to the paranasal sinuses, where retained mucosa in an obliterated cavity risks mucocele formation, mucocele development in the obliterated temporal bone is exceedingly rare (34,35). Many of these patients have sclerotic mastoids and extensive bone removal may add unnecessary time and risk to the operation.…”

“…Ihr Auftreten im Mastoid (▶abb. 13) ist sehr selten, wobei in der Literatur fast ausschließlich Fallberichte existieren [271][272][273][274][275][276][277][278][279]. Im Falle von knöchernen Erosionen und Destruktionen am Mukozelenrand kann es zu intra-und extrakraniellen Komplikationen kommen [280].…”

“…Mucoceles show signs of chronic sterile infection and have the ability to cause bony remodelling or reabsorption. Mucoceles may be primary or secondary to chro-nic inflammation, trauma, scarring from previous surgery or radiotherapy and, less common, to neoplasia [268][269][270]. Their occurrence in the mastoid (▶Fig.…”
