Primary malignant lymphoma of the gallbladder: a case report and literature review

Abstract: Primary malignant lymphoma of the gallbladder is extremely rare and the associated radiological findings are not well described in the literature. We report a case of malignant lymphoma in the gallbladder wall of a 78-year-old woman. Pre-operative CT and MRI showed homogeneous submucosal thickening of the gallbladder wall with a preserved mucosal surface. These unique radiological findings may be useful for diagnosing malignant lymphoma of the gallbladder.

“…These findings were compatible with the histological findings of the absence of clear pathological lesions in the mucosal epithelium and preserved normal structure of the biliary tract system with many capillaries in the mucosal layer (corresponding to the inner layer on CEUS) and fewer capillaries than that in the mucosal layer in the subserosal layer, and these two layers were bordered by the fibromuscular layer containing plenty of collagenous fibers (fibromuscular and deeper layers were corresponding to the outer layer). These findings may represent the characteristic pattern of infiltration by extranodal lymphomas, i.e., infiltration of surrounding tissue without destruction of the existing tissue structure [12,13]. In the case of our patient, the layered enhancement pattern of lymphomas of the extrahepatic bile duct was only observed on CEUS in the high-resolution power Doppler mode.…”

“…Some options were mentioned, especially fine-needle aspiration. Furthermore, half of the previously reported cases had gallstones [7]. Differential diagnosis includes various diseases.…”

“…To date, only 27 cases of primary malignant lymphomas involving the gallbladder have been reported in the literature. Of these, most are of the mucosaassociated lymphoid tissue (MALT) or diffuse large B cell (DLBCL) types [1]. Only two cases of primary follicular lymphoma of the gallbladder have thus far been reported [2,3].…”

“…Lymphoma of the gallbladder is extremely rare. Most reported cases of gallbladder lymphoma have been of the diffuse large B cell (DLBCL) or mucosa-associated lymphoid tissue (MALT) lymphoma types [1][2][3]. Primary follicular lymphoma of the gallbladder has only twice been reported [2,3].…”
