Primary Leiomyosarcoma of the Breast: A Case Report with Review of the Literature

Abstract: Breast sarcomas account for about 1% of all malignant breast cancers. Leiomyosarcoma (LMS), one of the rarest, was first described 20 years ago, and yet few published reports exist. A case of primary LMS in a 25-year-old woman is presented and is only the 18th well-documented case in the literature. The clinical presentation, diagnosis, therapy, and pathologic features are reviewed.

“…Carcinoma with sarcomatoid features, also known as metaplastic carcinoma is an important differential diagnosis for these tumours. Breast sarcomas account for about 1% of all malignant breast tumours of which leiomyosarcoma is a rare entity [1,2]. This tumour can present either as a primary or a metastatic lesion in the breast.…”

“…Metastasis to the breast is rare, with only four cases published in the literature to date [5,6]. Primary leiomyosarcoma of the breast is also a very rare tumour and less than 20 well-documented cases have been reported in literature [1]. It presents as a firm lobulated mass and resembles clinically a malignant phyllodes tumour.…”

“…Leiomyosarcoma is observed as a dense, circumscribed, lobulated mass on mammography, showing features similar to phyllodes tumors or fi broadenomas as described in most reports. 13,17,[20][21][22][23]25,27 Furthermore, on ultrasonography, as reported in all the reports cited, [20][21][22]24,26,30 leiomyosarcomas are either oval or lobulated tumors with a smooth clear border, mimicking a fi broadenoma, phyllodes tumor, or an intracystic papilloma. Therefore, with an image analysis alone, differential diagnosis between a leiomyosarcoma and a benign tumor can be extremely diffi cult.…”
