Primary Intradural Extramedullary Melanoma of the Cervical Spinal Cord

Abstract: Unlike most cases of primary intradural melanoma, this patient presented with unusual radiologic findings in the cervical spinal cord. The case described in the present study illustrates that primary spinal cord melanoma is rare and must be diagnosed with caution due to its variable clinical and radiologic presentation.

“…2 Clinically, PSM of extramedullary origin may be misdiagnosed as a neurogenic tumor (schwannoma), ependymoma, metastatic tumor, hematoma or meningioma. 3,[5][6][7][9][10][11][17][18][19] Ninety percent of melanomas found in the CNS are metastatic tumors. 8 A thorough physical examination is required to rule out melanoma in other organs outside of the spinal cord, before a diagnosis of PSM can be established.…”

“…Radiotherapy (from 30 to 60 Gy; median dose: 33.5 Gy) is effective for local control and is reserved for palliation of symptoms due to spinal cord compression or as an adjuvant to laminectomy. 5,6,10,11,17,18,21 Melanoma is also refractory to most systemic chemotherapies. Currently, it has not been confirmed whether postoperative radiotherapy or chemotherapy (such as interferon, methotrexate and temozolomide) has a significant role in overall survival time.…”

“…PSM was initially reported by Hirschberg in 1906, and accounts for 38% of all CNS melanomas (9). So far, fewer than 60 cases have been reported in the English literature (6,13).…”

“…Intermediate-grade melanocytic tumors are not easily classified as melanocytoma or melanoma as they present some histological features associated with aggressive behavior, and moreover, only sporadic cases have been reported in the literature (2,3). To the best of our knowledge, only two cases of multifocal primary melanocytic tumor have been reported in the literature (1,9). In this study, we present a case of multiple spinal cord melanoma located at the level of the cervical and upper thoracic vertebrae, and conduct a review of the pertinent literature.…”
