Primary hyperparathyroidism: an update

Abstract: Parathyroidectomy is the treatment of choice for patients with symptomatic PHPT and patients with asymptomatic PHPT who fulfill certain age, bone density, serum calcium, and creatinine criteria. Further research is needed to determine whether patients with prominent cardiovascular and neuropsychiatric manifestations would benefit from such intervention.

“…But in 1925, Mandl was the first to remove a parathyroid tumor with significant improvement of the disease [1,2]. Primary Hyperparathyroidism (PHPT) affects at least 1 in 1000 persons and its incidence rises with age, with a peak in the seventh decade of life [3,4]. It is the third most common endocrine diagnosis [3,4].…”

“…1 Since that time the mnemonic ''stones, bones, abdominal groans, thrones, and psychiatric overtones'' were used to describe its manifestations. 2 Hyperparathyroidism classically produced osteitis fibrosa cystic and pathological fractures. However, this is not the common presentation of the disease nowadays.…”

“…Furthermore, a newer entity of the disease is being identified, although its natural history is not fully understood, which is normocalcemic primary hyperparathyroidism. 2 Parathyroid hormone mediates its effect through cAMP as a second messenger. The hormone acts on PTH-1R receptor, which is present mainly on osteoblasts and in fewer amounts on osteoclasts.…”
