Cecal volvulus is a rare cause of intestinal obstruction caused by axial twisting of the cecum that occurs in 1–1.5 % of all intestinal obstruction, with an incidence of 2.8–7.1 cases per million annually. Cecal volvulus is potentially life-threatening without prompt surgical intervention. A 57-year-old woman presented with severe abdominal pain and distention. Laboratory examinations revealed normal white blood cell count with neutrophilic predominance. Diagnosis of acute cecal volvulus was made from a “whirl sign” on abdominal computed tomography. An exploratory laparotomy confirmed the diagnosis of cecal volvulus and a segmental ileocolic resection with primary anastomosis was carried out. The patient was discharged improved and returned to her normal activities of daily living. Conclusion: The management of volvulus involves a series of stepwise decisions that must be performed rapidly. Due to the fact that this condition is relatively uncommon makes it more critical for surgeons to maintain a solidified algorithm tailored to the disease process in order to amplify chances of successful outcome. In this case, the diagnosis of acute cecal volvulus was made from a “whirl sign” on abdominal computed tomography. An exploratory laparotomy confirmed the diagnosis of cecal volvulus and a segmental ileocolic resection with primary anastomosis was carried out. The patient was discharged, improved. Introduction While appendicitis is considered one of most common acute surgical conditions, several studies have reported abnormal histopathological findings in appendectomy specimens; however, sending all appendices to histopathology is not yet routinely done.Here we report many unusual findings. Those unusual findings played a role not only in confirming acute appendicitis as a cause of the presentation in some cases but also discovering etiologies that mimic it with great impact on its management. Methods Between January 2011 and December 2017, a total of 1510 patients were operated with appendectomy for a primary diagnosis of acute appendicitis. Among them, a total of 72 patients had incidental histopathologic findings in association with acute appendicitis or other pathologies instead of acute appendicitis. A retrospective analysis for those 72 patients was performed with all data being retrieved from the electronic health record system. Results Patients ages ranged between 4 and 71 years with a mean age equal to 23.1 years (SD = 14.2). Majority of patients were women (n = 52; 72.2%). Sixty of the seventy-two cases were seen in patients with negative appendectomies (n = 333) with an overall rate of 18% among this group of patients. The remaining 12 patients had additional findings in histopathology specimens beside acute appendicitis (n = 1131) with an overall rate of 1%. The most commonly reported pathologies were serositis, ovarian cysts, and Enterobius vermicularis in descending frequency. Conclusion Identification of unusual histopathological findings during microscopic examination of resected appendices is more common in female patients and in patients with negative appendectomy. histopathologic assessment of specimens will allow detection of congenital, infectious or malignant pathologies that mimic acute appendicitis clinically even in the absence appendicitis microscopically.

Aim: To evaluate role of thiol and disulfide homeostasis, a novel marker of oxidative stress, in the follow-up acute abdomen. Material and Method: This prospective study included 107 patients (62 men and 45 women) with a diagnosis of acute abdomen (AA). In all patients, age, gender and cause of acute abdomen were recorded. In addition, native thiol (-SH), total thiol (tSH) and disulfide (-S-S-) levels at baseline, admission and on postoperative day 3 were prospectively recorded and -SS-/-SH, -S-S-/tH and -SH/tSH ratios were calculated. Results: When the causes of acute abdomen were assessed, it was seen that 72 patients (67.29%) underwent surgery due to appendicitis while 5 patients (4.67%) due to lower GIS perforation, 7 patients (6.54%) due to perforated peptic ulcer, 5 patients (4.67%) due to sigmoid volvulus, 4 patients (3.74%) due to strangulated hernia and 14 patients (13.08%) due to miscellaneous reasons. When thiol and disulfide levels were assessed as a single parameter, mean thiol level was 316.71±78.16 (327.5) at preoperative period and 264.00±72.85 (278.30) at postoperative period.
