Primary cardiac lymphoma: diagnostic tools and treatment challenges

Abstract: Primary cardiac lymphoma (PCL) is a rare malignancy and the optimal treatment strategy remains uncertain. It appears to respond much better to systemic chemotherapy than to surgery and it should be considered in the differential diagnosis of all cardiac tumours before definitive management is undertaken. We report a case of this rare disorder treated successfully with a combination of rituximab and cyclophosphamide, adriamycin, vincristine and prednisolone. The patient developed recurrent unstable ventricular … Show more

“…Arrhythmias are frequent, occur early in the course of the disease, and may contribute significantly to early death [5, 8, 17, 25]. Reports of AICD use in patients with PCL are scarce and usually follow development of life-threatening arrhythmias [26, 27]. Our patient preemptively received prophylactic AICD, which may explain her long survival of 87 months.…”

“…Three PCL cases with extracardiac involvement have previously been reported. Two cases presented with an extracardiac metastasis at the first diagnosis 1,2. One case had a PCL involving only the heart at the primary diagnosis, but microscopic involvement of the lungs, peritoneal, and retroperitoneal adipose tissue was detected on autopsy 3.…”

“…Traditionally, PCL is defined as an extranodal lymphoma limited to the heart or pericardium. However, some previous reports demonstrated PCL cases with extracardiac involvement 1,2,3. PCL occupies 1.3% of primary cardiac tumors and 0.5% of extranodal lymphomas 4.…”

“…In view of the high risk of heart failure, other procedures could not be performed. There are two descriptions of a similar diagnostic path; in both cases a mediastinoscopy yielded the diagnosis [ 7 , 8 ]. Some procedures, such as an open-heart biopsy, have 100% sensitivity but are associated with a high risk of serious complications in patients with uncontrolled heart failure [ 4 , 7 ].…”
