Childhood maltreatment involves dynamics between the type of maltreatment experienced and the context of maltreatment. Reports of alleged maltreatment to child protective services may overlap and shift over time, complicating understanding of their independent and interacting nature, including how child protection systems respond. Latent class analysis (LCA) and latent transition analysis (LTA) were used to construct data-based models of longitudinal dynamics of alleged maltreatment throughout childhood. We sought to identify patterns leading to system decisions to substantiate allegations of maltreatment and place children in foster care. Using linked birth and child protection records, we defined a cohort of children born in California in 1999, 29.4% of whom had at least one referral for alleged maltreatment before their 18th birthday. Maltreatment and perpetrator indicators were coded, and LCA identified five alleged maltreatment classes and one class of children without referrals. LTA determined consistency of classifications and estimated transitions between classes over age periods. Children with multitype maltreatment patterns or experiences of neglect were most likely to experience future maltreatment allegations. Estimated probabilities of placement indicated children with Multitype Maltreatment allegations were more likely to experience substantiated maltreatment allegations and out-of-home placements. Findings identify a repeatable method for better understanding complex systems.

Introducción. Este estudio explora el conocimiento de estudiantes, profesorado y personal no docente sobre los factores de riesgo del maltrato infantil y su grado de sensibilización comparándolo con otros ámbitos profesionales y formativos. Se mide el grado de conocimiento sobre los factores de riesgo de maltrato relacionados con las características del niño, los factores familiares, socioeconómicos y culturales, y los factores individuales de la persona que comete el abuso. Se midió la sensibilidad al maltrato a través de la percepción de la gravedad de diferentes conductas relacionadas con el maltrato pasivo (negligencia) y activo y la frecuencia de las distintas conductas abusivas. Método. El procedimiento de muestreo realizado fue no probabilístico por conveniencia. Participaron 878 personas (68,3% mujeres) entre 18 y 65 años. Se diseñaron instrumentos específicos utilizando una escala tipo Likert de 5 puntos para evaluar el grado de conocimiento sobre los factores de riesgo de maltrato y la sensibilidad ante el mismo. Para el análisis de datos se utilizó el programa SPSS v.25.0 y se realizaron análisis de varianza multivariante y correlaciones de Pearson. Resultados. El estudio muestra que los docentes parecen estar más preparados que la población general para la detección ya que tienen un mayor conocimiento de los factores de riesgo y la frecuencia y gravedad del abuso. Tanto el conocimiento sobre factores de riesgo en la aparición de maltrato como la experiencia personal previa parecen ser factores que influyen en la sensibilidad ante el maltrato. Discusión y conclusiones. Se resalta la importancia de la sensibilidad y formación ante el maltrato infantil y se concluye la necesidad de que los programas preventivos se planifiquen teniendo en cuenta el perfil al que se dirigen por las diferencias de conocimiento de unos grupos y otros.

Introduction: International studies suggest that a lack of forensic knowledge, clinical skills and organizational support may hinder nurses' implementation of the duty to report suspected cases of maltreatment to child welfare services. No studies to date have documented how the duty to report is integrated into primary care nursing practice. Objectives: To describe primary care nurses' perceived role in preventing child maltreatment and to explore the process of implementing mandatory reporting to child welfare services. Methods: A situational analysis was conducted using 14 semi-structured interviews with primary care nurses in Quebec, Canada. These interviews were qualitatively mapped and analyzed using inductive coding. Results: Clinical vigilance is an important dimension of nursing practice, which is influenced by external constraints. The implementation of the duty to report relies on clinical intuition, the exercise of discretionary power and organizational work that can be preventive, targeted or symbolic in scope. The determining factor in the implementation of the duty to report is the exercise of control over the situation and its anticipated consequences. Discussion and conclusion: These findings provide insight into the process of implementing mandatory reporting practices to child welfare services in terms of factors that may influence the exercise of nurses’ discretionary power.
