Background The aim of this trial was to comparatively evaluate Icon® resin infiltration and Clinpro™ XT varnish in restoring aesthetics of white spot lesions (WSLs) present post-orthodontic treatment. Materials and methods Two hundred forty WSLs were detected in 193 teeth of 12 patients. The participants were analysed—before intervention (T 0 ), immediately after intervention (T 1 ), 3 months later (T 2 ) and 6 months later (T 3 ), with a 1:1 allocation ratio for the application of Icon® resin infiltration and Clinpro™ XT varnish. Using a computer-generated allocation sequence, block randomization was done. A spectrophotometer was used to assess the colour of WSLs and the adjacent enamel, while a DIAGNOdent® was used to assess the fluorescence loss. Results Immediately after the intervention, Icon® resin infiltration showed statistically significant better improvement than Clinpro™ XT varnish in restoring the colour ( p = 0.000); however, at 3 ( p = 0.001) and 6 months ( p = 0.000), this was reversed. Except at 3 months, the fluorescence loss sequentially reduced more for Icon® resin infiltration (4.48 ± 1.42 at T 0 to 1.48 ± 0.81 at T 3 ) and was not statistically significant. Conclusions Clinpro™ XT varnish showed significantly better improvement than Icon® resin infiltration in restoring the colour and lightness of the WSLs at 3 and 6 months. The fluorescence loss significantly recovered with both intervention methods between immediate application and at 6 months. However, Clinpro™ XT varnish-treated WSLs showed a statistically significant difference compared to the adjacent sound enamel at 6 months.

The most frequent iatrogenic problem in orthodontics is white spot lesions (WSLs). Maintenance of an adequate and constant presence of fluoride ions in the vicinity of the enamel on the periphery of bracket bases helps protect against the development of WSLs. Resin-modified glass ionomer cements (RMGICs) minimize the development of WSLs due to their continuous fluoride uptake from the environment and re-release, but the perception that orthodontic brackets bonded with RMGICs frequently fail due to low initial bracket shear bond strength persists. This perception is correct if the clinician were to bond these materials as recommended by the manufacturers, which includes conditioning the enamel with an extremely weak 10% polyacrylic acid instead of the traditional 37% phosphoric acid etch. In order to successfully use RMGICs, it is recommended to first deproteinize the enamel surface by applying 5.25% sodium hypochlorite for 1 min to remove the acquired dental pellicle (which impedes proper etching of the enamel surface), followed by a 15-to 30-s etching with 37% phosphoric acid (so the resulting etch patterns on the enamel surface are types I and 2, not type 3, thereby increasing bracket bond strength), and followed by moistening the enamel surface to further increase this strength. In following these recommendations, the clinician will minimize the risk to patients of developing lifelong WSLs and their consequences. In the final analysis, the degree of damage provoked by WSLs is vastly more significant on the health of the enamel than the bonding and debonding process. It is the author's hope that this review of the scientific literature will help clinicians achieve the best results for their patients.

Tooth root resorption: A review

Tooth root resorption is multifactorial, leading to progressive destruction and eventual loss of tooth root dentin and cement. There are internal and external types of root resorption, each having its variety. The etiology and pathogenesis of tooth root resorption are poorly understood, and the most significant etiological factors are trauma, pulpal infection, tooth bleaching, and orthodontic treatment. Tooth root resorption is primarily asymptomatic; thus, it is revealed accidentally by radiographic examination. Progressive clinical manifestations are pain, tooth discoloration, tooth mobility, and other conditions. Awareness of the causes and risk factors allowing tooth root resorption, and regular radiographic examination, in case of necessity, make it possible to reveal resorption at an early stage and to prevent its further development. Thus, the aim of this study is to present etiopathogenesis, a clinical course, and diagnostic peculiarities of internal and external types of tooth root resorption, enabling practicing dentists to timely diagnose root resorption and take appropriate measures to avoid further complications. Within the limitation of this review, even though the etiopathogenesis of tooth root resorption is yet not fully understood, it is suggested that the etiological factors fall into two groups (endogenic and exogenic) to enhance further understanding of the possible causes and mechanisms of root resorption and allow practitioners to monitor high-risk patients and make timely diagnoses. Moreover, radiographic examination and CBCT are indispensable for the diagnosis of root resorption.
