A grande migração de chagásicos para os centros urbanos, especialmente a partir dos anos 70, aliada ao combate sistemático ao vetor da doença em vários países da América Latina, fez com que a transmissão transfusional se tornasse, nas décadas de 80 e 90, o principal mecanismo de transmissão da doença de Chagas.
1,2Levantamentos da Organização Pan-americana de Saú-de (OPAS), comparando dados sobre a doença de Chagas de 1990 e 2006 na América Latina, mostram que o número de infectados reduziu de 30 milhões para 7,7 milhões, população de risco de 100 milhões para 28 milhões e a incidência anual de novos chagásicos de 700 mil para 41 mil.
3Estes fatos refletiram diretamente sobre a queda da prevalência da infecção chagásica nos doadores. Assim é que, enquanto a prevalência sorológica na América Latina e no Brasil na década de 70 era de 6,06% e 11,08%, respectivamente, em 2006 foi de 1,28% e 0,21%.
4Em Uberaba, a redução na soroprevalência no período foi de 16,02% para 0,14%. publicado neste fascículo, também demonstra importante queda da infecção chagásica nos doadores de Pernambuco, de 4,4% em 1970, para 0,17% em 2007. A menor prevalência abaixo dos 30 anos, observada pelos autores, é consenso na literatura e reflete os resultados do combate sistemático ao vetor ocorrido no Brasil a partir dos anos 70. Contudo, a maior frequência no gênero masculino (78,36%) possivelmente é explicada pela menor participação das mulheres que representam no Brasil e no nordeste, respectivamente, 34,3% e 29,0%. Em contraposição a estes resultados, observamos em Uberaba (MG), frente a uma frequência de doação feminina de 27%, um índice de inaptidão de 0,4% verso 0,28% para os homens. 4 Outro fato que atesta a eficácia do combate ao vetor em diversos países da América Latina é a queda acentuada da prevalência sorológica em crianças entre os anos 80 e 90. Na Argentina foi de 82,1%, no Brasil, de 99,1% (18,5% para 0,17%), no Chile de 77,8% e no Paraguai de 58,1%. Portanto, a redução drástica da transmissão vetorial e consequentemente da prevalência de doadores chagásicos, aliada ao aumento da cobertura sorológica em bancos de sangue, resulta em sensí-vel diminuição do risco da doença de Chagas transfusional. 10 Todavia, enquanto a América Latina caminha célere para o controle da endemia, o fluxo migratório dos países endêmicos para a América do Norte, Europa, Japão e Austrália vem provocando inquietudes devido ao risco evidente e já comprovado de transmissão transfusional e vertical da infecção chagásica, levando-os a submeter seus doadores à triagem sorológica para doença de Chagas. Contudo, paralelamente à baixa prevalência de doadores chagásicos, vem chamando atenção a significativa proporção de doadores inaptos sorológicos em decorrência de sorologia inconclusiva.
