Prevalence of subthreshold forms of psychiatric disorders in persons making suicide attempts in Hungary

Abstract: The subthreshold definitions in this study included only those forms of the disorders which required the same duration as the criteria DSM-IV disorder with fewer symptoms. Conclusions - Suicide attempts showed a very high prevalence of subthreshold disorders besides psychiatric disorders meeting the full criteria required according to the DSM-IV. Subthreshold forms of mental disorders need to be taken into account in suicide prevention.

“…In a recent study conducted in 140 consecutive attempters in Hungary, Balazs et al found 84% with an Axis one diagnosis and 79% with a subthreshold diagnosis; in only 8% was none present. Co-morbidity between full and/or subthreshold Axis one disorders was 69% [2] This points out that the prevalence of co-morbid psychiatric diagnoses is more frequent than previously described in suicide attempters.…”

“…Comorbidity, mainly anxiety disorders, increases the risk of suicidal behaviour among adolescents (Wunderlich, Bronisch, & Wittchen, 1998). Balázs, Bitter, Lecrubier, Csiszér, and Ostorharics (2000) found that almost two thirds of adult suicide attempters had MDE, half had GAD, one tenth had subthreshold‐depressive episode and one fifth had subthreshold‐GAD. Only a few adolescent studies have focused on subthreshold mental disorders, including subthreshold‐depressive episodes and GAD, as a possible risk factor for suicide.…”

“…The MINI and MINI Plus are used worldwide, there are several nationally standardized versions (Italian, Japanese and Arabic [84][85][86] ). The interview is favoured in establishing diagnoses and comorbidities in affective disorders especially including a separate module of 'suicidal behaviour' [87][88][89][90] . Although this instrument was not primarily designed to elicit information about SB, it does provide detailed and useful data about its past and present occurrence.…”
