Prevalence of obstructive sleep apnoea in acute coronary syndrome: Routine screening in intensive coronary care units

Abstract: A systematic screening of patients in the ICCU is acceptable. OSA is frequently found in the acute phase of ischaemic heart disease and its presence is associated with more severe ACS and a poorer left ventricle systolic function.

“…We found a high frequency of OSA in our sample assessed via polysomnography within one month of AMI, with 85% of patients having an AHI ≥ 5/h and 49% having moderate to severe OSA. These results were in agreement with the large prevalence of OSA in this condition [ 9 , 10 ]. Moreover, 40% of AMI patients displayed a ventricular remodeling characterized by an increase of ≥12% of LVEDV [ 19 ].…”

“…Identifying the risk factors of post-AMI ventricular remodeling is thus a critical step in research aiming to improve the patient’s prognosis in AMI. Obstructive sleep apnea (OSA) is a sleep-related breathing disorder that increases the risk of cardiovascular diseases [ 7 , 8 ] which is very common in more than half of patients with AMI [ 9 , 10 ], and may be a risk factor of post-AMI ventricular remodeling.…”

“…In heart failure patients, OSA was associated with a slight decrease in LVEF [34]. Morra et al reported a statistically significant inverse correlation between the AHI and LVEF [29]. Nakashima et al observed that in ACS patients with equivalent LVEF at baseline, after 21 days, the LVEF was significantly lower in patients with OSA vs. control subjects (52% vs. 59%; p < 0.02) [2,21].…”

“…Of these patients prescribed CPAP, the compliance at 30-day follow-up was 45%. Compliance was associated with a reduced 30-day hospital readmission for cardiac issues compared to those who were non-compliant with CPAP (partial and no usage) [29]. After admission -PSG and CPAP prescribed.…”
