Prevalence of Integrase Strand Transfer Inhibitors (INSTI) Resistance Mutations in Taiwan

Abstract: Antiretroviral therapy containing an integrase strand transfer inhibitor (INSTI) plus two NRTIs has become the recommended treatment for antiretroviral-naive HIV-1-infected patients in the updated guidelines. We aimed to determine the prevalence of INSTI-related mutations in Taiwan. Genotypic resistance assays were performed on plasma from ARV-naïve patients (N = 948), ARV-experienced but INSTI-naive patients (N = 359), and raltegravir-experienced patients (N = 63) from 2006 to 2015. Major INSTI mutations were… Show more

“…The fact that only 0.6% of samples analyzed had INSTIs major RAMs suggests that there will be a lesser likelihood of resistance to INSTIs in Cameroon and that INSTIs will be effective for HIV-infected Cameroonians. Similar trends were observed in other countries in Africa [13,34,[43][44][45], Asia [46][47][48][49][50][51], Europe [52][53][54], and South America [55] where studies showed a low frequency of INSTIs major RAMs.…”

“…However, INSTI polymorphic substitutions were not rare, and those who had INSTI polymorphic substitutions were more likely to have a higher HIV viral load and syphilis infection. The absence of INSTI resistance-associated mutations in this study is similar to the CORONET study [ 9 ], the Italian Cohort of Antiretroviral Naive study group [ 15 ], the National Taiwan University Hospital Study [ 8 ] and the epidemiological surveillance study of the European SPREAD programme in 2006–2007 [ 5 ], in which primary resistance to INSTIs was rare.…”

“…Our study could not be compared to those reported from northern Taiwan [ 3 , 8 ]. In the study of Lai et al [ 3 ], they reported that the prevalence of INSTI mutation was 3.2%.…”

“…Genotypic resistance assays were neither offered free-of-charge or routinely determined before cART initiation, although surveillance data suggested that the prevalence of transmitted drug resistance of HIV-1 to at least one antiretroviral was in the range of 10–15%; instead, the assays were performed at Taiwan CDC or a few designated hospitals for patients with virological failure and PVL >1000 copies/mL [1214]. The national HIV treatment guidelines had increased the CD4 cell count threshold for cART initiation from 350 to 500 cells/mm 3 in September 2013 [11], which was further revised to treat all HIV-positive patients irrespective of CD4 cell count in June 2016.…”
