BackgroundHemorrhagic fever with renal syndrome (HFRS) and leptospirosis are seasonal rodent-borne infections in the Republic of Korea (Korea). The occurrences of HFRS and leptospirosis are influenced by climatic variability. However, few studies have examined the effects of local climatic variables on the development of these infections. The purpose of this study was to estimate the effect of climatic factors on the occurrence of HFRS and leptospirosis in Korea.MethodsDaily records on human cases of HFRS and leptospirosis between January 2001 to December 2009 were analyzed. The associations of climatic factors with these cases in high incidence provinces were estimated using the time-series method and multivariate generalized linear Poisson models with a maximal lag of 12 weeks.ResultsFrom 2001 to 2009, a total of 2912 HFRS and 889 leptospirosis cases were reported, with overall incidences of 0.67 and 0.21 cases per 100,000, respectively, in the study areas. The increase in minimum temperature (1 °C) at a lag of 11 weeks was associated with 17.8% [95% confidence interval (CI): 15.1, 20.6%] and 22.7% (95% CI: 16.5, 29.3%) increases in HFRS and leptospirosis cases, respectively. A 1-h increase in the daily sunshine was related to a 27.5% (95% CI: 18.2, 37.6%) increase in HFRS at a lag of 0 week. A 1% increase in daily minimum relative humidity and a 1 mm increase in daily rainfall were associated with 4.0% (95% CI:1.8, 6.1) and 2.0% (95% CI: 1.2, 2.8%) increases in weekly leptospirosis cases at 11 and 6 weeks later, respectively. A 1 mJ/m2 increase in daily solar radiation was associated with a 13.7% (95% CI: 4.9, 23.2%) increase in leptospirosis cases, maximized at a 2-week lag.ConclusionsDuring the peak season in Korea, climatic factors play a significant role in the development of HFRS and leptospirosis. BackgroundYeoncheon is an endemic region for hemorrhagic fever with renal syndrome (HFRS) and has been reporting HFRS cases intermittently in other seasons, including autumn. This study was conducted to determine whether a seasonal variation pattern of HFRS exists in Yeoncheon.Materials and MethodsFrom 2002 to 2016, raw data of the number of patients with HFRS in Yeoncheon and nationwide was collected from the Korea Center for Disease Control and Prevention. On the basis of the raw data, the incidence per 100,000 population was calculated for each month of the year. The twelve months were divided into four quarters, and the proportion of the disease by each quarter was calculated. The effects of sex, age, quarter, and time on HFRS occurrence were analyzed by Poisson regression analysis.ResultsA total of 6,132 HFRS cases occurred nationwide, and 62 cases occurred in Yeoncheon. The incidence of the disease in Yeoncheon (9.07/100,000) was statistically higher than that nationwide (0.81/100,000). The quarterly incidence showed that occurrence proportion of HFRS was high in the third and fourth quarters (12.9%, 67.5%) nationwide, whereas it was relatively similar in all quarters in Yeoncheon (17.7%, 21.0%, 25.8%, 35.5%). The Poisson regression model showed that the relative risk of HFRS nationwide was 1.322 in the third quarter and 6.903 in the fourth quarter, but Yeoncheon had no risk increase by quarter.ConclusionIn this study, HFRS in Yeoncheon demonstrated no seasonal variation pattern compared to that in nationwide Korea, which may be considered a regional characteristic. Furthermore, in other regions where HFRS is endemic, like Yeoncheon, HFRS may arise regardless of seasonal variations.

This study describes the epidemiology of hemorrhagic fever with renal syndrome (HFRS) in the past 10 yr (2001)(2002)(2003)(2004)(2005)(2006)(2007)(2008)(2009)(2010) in Korea. During this period, a total of 3,953 HFRS patients and an average prevalence rate of 0.81 per 100,000 population were recorded, with a total of 40 fatal cases, corresponding to a case fatality rate of 1.01%. More HFRS cases were found in men than in women (57% vs 43%), and a higher prevalence rate of HFRS was observed in patients older than 40 yr (82.1%). The highest numbers of HFRS cases were found amongst farmers (35.6%). The majority of HFRS cases (71.3%) occurred in the last quarter of the calendar year (October to December). More HFRS cases occurred in the western part than in the eastern part of Korea (68.9% vs 31.1%). The incidence of HFRS was significantly higher (P < 0.001) in rural areas than in urban areas (80.3% vs 19.7%). The viruses are transmitted to humans by the inhalation of excreta of rodents infected with hantaviruses (3). HFRS was first recognized during the Korean War in the early 1950s, and although a hantavirus vaccine has been developed, the disease remains a serious problem in Asia and Europe (1, 4), and especially among soldiers in Korea (5, 6). A 2006 report describes HFRS epidemiology in endemic regions in Korea during a relatively short period, 1995-1998 (7); however, an updated epidemiological study is necessary to better understand the current status of HFRS in Korea. This study provides a comprehensive epidemiological overview of HFRS during the past decade in Korea.Raw data were utilized from the National Notifiable Disease Surveillance System (NNDSS) website of the Korean Center for (Table 1). The prevalence rate (PR) of HFRS per 100,000 population and case fatality rate (CFR) were estimated by the criteria established by the World Health Organization (WHO), and the upper and lower limits of the 95% confidence interval (CI) were calculated. Statistically significant differences between the epidemiological aspects were determined at P < 0.05 and P < 0.01 using the chi-square test or the paired t-test. The data analyses were carried out using the statistical system software included in Microsoft Excel 2007.As shown in Table 1, the HFRS cases were analyzed according to the PR, CFR, gender, age, and occupation of the infected individuals.
