Objectives:To investigate the prevalence of caries and fissure sealants on the first permanent molars (FPMs) among 6–9-year-old girls in Riyadh, Saudi Arabia.Materials and Methods:The FPMs of 17,891 school girls from 120 randomly selected public primary schools were evaluated by visual and tactile examination for the carious status and the presence of fissure sealants. Chi-square test was used to analyze the data at level of 5%. Results:A total of 58,140 FPMs were assessed in the 17,891 children. Nearly 64.6% of the children were caries free. Only 1.3% of the children had at least one fissure sealant applied. At tooth level, the decayed FPMs counted for 24.6%. There was obvious underutilization of fissure sealants on the FPMs; 0.8% (n = 478). The caries prevalence in the mandibular FPMs (33%) was significantly higher than in the maxillary FPMs, 18.2% (P < 0.01). The proportion of carious FPMs increased with age of the children significantly (P < 0.01). There was no significant difference in the proportion of the presence of fissure sealants among the three different grade/age groups (P > 0.05).Conclusions:Caries prevalence in the FPMs was high and serious among this cohort of young students. This was contrasting the very low prevalence of fissure sealants.

The first permanent molar (FPM) plays an essential role in the masticatory function by contributing to the implementation and the maintenance of the occlusion. However, it is considered as the most frequently affected and the earliest affected tooth by caries; 27.4% of the 6-8 years old children have developed at least one cavity on one of the four first permanent molars, according to a study conducted among 3276 school children in Casablanca .Therefore, the FPM should benefit from special vigilance on the part of the practitioner to ensure that any early carious lesion is intercepted. In addition, the FPM, due to its period of mineralization coinciding with early childhood diseases, can erupt with a structural abnormality. Molar incisor hypomineralization (MIH) is considered to be the most common defects observed on first permanent molars among children. A study conducted among 1077 children aged 7-10 years enrolled in schools in Casablanca showed that 7.9% of children were affected with MIH. About 84.7% of the children had the four molars affected. Children with HIM had a significantly higher prevalence of caries: 78.8 versus 33.5%.These structural abnormalities of the enamel must be carried out earlier to ensure that the coronary anatomy is the least compromised.

Background: Pit and fissure sealants are one of the most effective preventive treatment for dental caries in children. Knowledge of prophylactic treatments like sealants among parents and teachers play major role in prevention of dental caries. Objectives: To assess the awareness of sealants among parents and teachers of the participating children and to assess the extent of sealant use among private dental practitioners. The study also assess the presence of sealants in the study population. Methodology: 949 school children aged 6-14 years were selected from 20 different schools. The children were selected using multistage sampling technique. Closed ended questionnaires were provided to all participant children parents. Similar questionnaires were also provided to the 274 teachers working in the selected schools. The responses from these questionnaires were used to assess the awareness and knowledge of parents and teachers regarding pit and fissure sealants. 120 private dental practitioners practicing in Kannur district were also provided with questionnaires to assess their knowledge and attitude regarding pit and fissure sealants and also to assess the extent of use of sealants in their practice. The results obtained from the survey was tabulated and statistically analyzed using chi square test. Results: The results showed that 2.85% of the examined children had pit and fissure sealants in their oral cavity. Out of the 857 parents only 8.63% of them reported prior knowledge about sealants. Among the 274 teachers surveyed 15.7% of teachers reported prior knowledge about sealants. Among the 120 private clinicians who participated almost 25% reported that they use dental sealants in their practice. Interpretation and conclusion: The present study revealed that the awareness of pit and fissure sealants among parents and teachers is low and the frequency of sealant use by private practitioners was also noted to be low.
