Prevalence of Candida species in hospital-acquired urinary tract infections in a neonatal intensive care unit

Abstract: Candida spp. were the pathogens identified in 42% of hospital-acquired urinary tract infections in a neonatal intensive care unit. Candidemia was associated with 52% of candidal UTI and bacteremia with 8% of bacterial UTI. Candidal UTI occurred significantly earlier than bacterial UTI. Renal fungus balls were present in 35% of infants with candidal UTI.

“…This finding suggests that serial brain imaging may be valuable in cases of persistent candidemia, as others have described for RUS imaging. 17,31 Ophthalmologic involvement in neonates with candidemia has been reported in 0% 7,24 to 6% 23 of patients who underwent examinations. These cases demonstrate the importance of ophthalmologic examination in neonatal candidiasis, but currently available data are restricted primarily to clinical series, thus firm recommendations regarding timing of the examination cannot be made.…”

“…When a Candidal urinary tract infection (UTI) is diagnosed in a neonate, it is important to assess the patient for candidemia. Phillips and Karlowicz found candidemia in 13 of 25 Candidal UTIs in neonates and Bryant et al reported this condition in 12 of 36 cases (7,8). Renal involvement in neonatal candidemia varies from 5% to 33% (9-11).…”

“…While a bag specimen for urine culture in neonate is not reliable, the growth of more than 103 colony forming units/mL (CFU/ML) in a urine culture gathered by suprapubic aspiration or greater than 10 4 CFU/mL in a catheter collected sample, there is Candidal UTI (7,16,17). Ultrasonography can find renal parenchymal involvement, mycetoma, fungal masses or obstructive effect of fungal mass in the urinary tract.…”
