Prevalence of and risk factors for dermatoporosis: a prospective observational study of dermatology outpatients in a Finnish tertiary care hospital

Abstract: Acknowledging the selection bias in our study, the prevalence of dermatoporosis in a dermatology consultation in Finland seems as frequent as in France. These results prompt us to weigh the indications of long-term corticosteroids use in frail elderly patients.

“…Yet, pruritus – the major cause for impaired QoL in SS – is notoriously difficult to treat 4,5 . In our case, dupilumab drastically improved pruritus, reduced the use of TCS and thus prevent TCS‐associated skin toxicity 16 . Indeed, prior to dupilumab, our patient was forced to overdose TCS to keep pruritus at tolerable levels.…”

Immediately after initiation of dupilumab, TCS use was reduced to doses that are considered safe.…”

“…Dermatoporosis was significantly associated with ultrapotent topical corticosteroids (odds ratio (OR) 5.34), oral corticosteroids (OR 3.22), concomitant corticosteroid therapy, anticoagulant and chronic renal failure (OR 4.02) while age had only a marginal impact (OR 1.05). Patients with bullous pemphigoid were those with the highest prevalence of dermatoporosis in their cohort (64%) [4].…”

“…Dermatoporosis is a condition that can lead to acute skin failure in the ICU. Dermatoporosis has a high prevalence in autoimmune bullous disorders such as pemphigoid, and bullous disorders are among the dermatological diagnoses that can lead to an ICU admission (Figure 4 and 5) [4], [17], [18].…”

“…1,2 Notably, in our patient, differently from other cases in the literature, 1,3 AHA occurred while the underlying PV was under clinical remission, raising We wish to highlight that AHA may represent a most challenging diagnosis: in our patient, the condition resembled dermatoporosis, a sign of cutaneous fragility commonly observed during topical and systemic corticosteroid therapy. 4 No systemic symptoms suggestive of a bleeding disorder occurred until the patient experienced the acute event leading to death. We agree with Wernham et al 1 that dermatologists should be aware of AHA as a potentially lethal, albeit rare, complication of AIBD.…”
