Purpose: Previous studies have used various definitions to classify chronic obstructive pulmonary disease (COPD) patients into chronic bronchitis (CB) and non-CB patients. This study was performed to identify differences among three definitions of CB based on the classical method, St. George's Respiratory Questionnaire (SGRQ), and the CAT (COPD Assessment Test) score. Patients and methods: We extracted data from the multicenter Korea COPD Subgroup Study (KOCOSS) cohort, for which patients recruited from among 47 medical centers in South Korea beginning in April 2012. Patients were classified according to three different definitions of CB: 1) classical definition; 2) SGRQ (using questions regarding cough and sputum); and 3) CAT score (comprising cough [CAT1] and sputum [CAT2] subscale scores). Results: A total of 2694 patients were enrolled in this study. The proportions of CB were 10.8%, 35.8%, and 24.0% according to the classical, SGRQ, and CAT definitions, respectively. The three definitions yielded consistently significant differences between CB and non-CB patients in modified Medical Research Council dyspnea scale CAT score, SGRQ score, number of moderate-to-severe exacerbations per year and forced expiratory volume in 1 second. By three definitions, CB consistently predicted future risk of exacerbation. The kappa coefficient of agreement between the classical definition and SGRQ definition was 0.29, that of the classical definition and CAT definition was 0.32, and that of the SGRQ definition and CAT definition was 0.44. Conclusion: Patients with CB according to the new definitions based on SGRQ or CAT score showed similar clinical characteristics to those defined according to the classical definition. The new CB definitions may be used as alternatives to the classical definition.

Резюме. Загострення хронічного бронхіту (ХБ) є фактором ризику смерті, госпіталізацій, погіршення респіраторних симптомів, обмеження фізичної активності та інвалідизації хворих. Причинами від 60 до 80 % всіх загострень ХБ складають інфекційні збудники. Мета роботи: вивчити спектр збудників інфекційного загострення ХБ та покращити ефективність лікування хворих з цією недугою. Матеріали та методи. Для вивчення збудників загострення проводили вірусологічне та мікробіологічне дослідження мокротиння, змивів або мазків із слизової оболонки носової порожнини у 100 хворих з інфекційним загостренням ХБ. У дослідженні ефективності лікування приймали участь 146 пацієнтів, яких було поділено на три групи в залежності від об'єму протиінфекційної терапії (І група-антибактеріальний препарат + мукорегулятор, ІІ група-антибіотик, мукорегулятор + інгаляції 0,02 % стерильного розчину декаметоксину, ІІІ група-мукорегулятор + інгаляції 0,02 % стерильного розчину декаметоксину). Оцінку ефективності лікування проводили на 3-5-й день (візит 2) та 10-14-й (візит 3) від його початку. Результати. При вивченні збудників загострення ХБ у 44,0 % випадках ідентифіковані віруси, у 35,0 %-бактерії, у 21,0 %-вірусно-бактеріальні асоціації. Під впливом лікування кількість хворих, у яких до 2 візиту зменшився сильний кашель, в І групі склала 9,5 %, в другій-10,5 %, в третій-5,6 %. Наприкінці лікування помірний кашель зберігався у 41,5 % хворих І групи, 7,0 % хворих ІІ групи та 22,2 % хворих ІІІ групи. До 3 візиту нормалізація температури тіла в групах спостереження була досягнута у 73,6 % хворих першої групи, 89,5 % хворих ІІ групи та 72,2 %-ІІІ групи. Гіперемія зіву та утруднене носове дихання у хворих ІІ групи до кінця лікування зменшилися у більшої кількості (у 54,4 %) хворих у порівнянні з хворими І групи (у 30,2 %), p < 0,05. Симптоми риніту до кінця лікування зменшилися у більшої кількості хворих ІІІ групи (у 52,7 %) порівняно з хворими І групи (у 32,1 %), p < 0,05. Загальна тривалість антибактеріальної терапії у пацієнтів 1-ї групи становила в середньому (7,8 ± 0,8) дні, а у хворих 2-ї групи-(5,7 ± 0,6) дні (p < 0,05). Висновки. Додаткове інгаляційне застосування 0,02 % стерильного розчину декаметоксину в комплексному лікуванні пацієнтів з інфекційним загостренням ХБ дозволило зменшити вираженість та тривалість (в середньому на 1-2 дні) проявів інтоксикації та катаральних явищ. Небулайзерна терапія 0,02 % стерильним розчином декаметоксину дозволяє скоротити тривалість інфекційного загострення ХБ в середньому на 1,6 дня, скоротити тривалість застосування антибіотиків на 2 дня, а також надає можливість уникнути невиправданого призначення антибактеріальних препаратів при нетяжкому перебігу інфекційного загострення ХБ. Ключові слова: інфекційне загострення хронічного бронхіту, декаметоксин, небулайзерна терапія.

BACKGROUND The aim of the study is to study the prevalence of obstructive lung diseases like COPD, Bronchitis and Bronchial Asthma in Community Health Center, Haliyal. MATERIALS AND METHODS This study was undertaken in Community Health Center, Haliyal. This was a prospective study. Duration of study was nearly one year from December 2015 to March 2017. All patients with symptoms of obstructive lung disease were involved in the study. Patients with COPD with history of pulmonary tuberculosis were also involved in the study. RESULTS During study period of nearly one year, 508 cases of obstructive lung disease were detected; out of which 312 were male while 196 were female patients. When compared to total patients examined during this period, the prevalence of COLD was 3.4%. Out of 508 cases, 406 cases were of chronic bronchitis, 95 patients of COPD and 7 patients had history of bronchial asthma. In our study, out of 508 patients 375 (74%) had history of smoking bidis and cigarette. Out of 406 chronic bronchitis patients, 5 had history of pulmonary tuberculosis, while out of 95 COPD patients, 7 had history of pulmonary tuberculosis i.e. out of 508 patients total 12 patients (1.4%) had history of tuberculosis. CONCLUSION Prevalence of COLD in our study was 3.4%. In our study, smoking was main risk factor, 74% of patients had history of smoking, while direct and indirect exposure to industrial products in sugarcane industry in Haliyal was second important risk factor in 375 patients. 12 patients who had previous history of tuberculosis progressed to development of COLD. Yet interstitial lung disease secondary to exposure to industrial (sugarcane) risk factors remains unconfirmed due to lack of facility and economical constraint as CT could not be done.
