Presymptomatic diagnosis of bronchogenic carcinoma associated with bilateral diffuse uveal melanocytic proliferation

Abstract: A 62-year-old man presented with bilateral diffuse uveal melanocytic proliferations (BDUMP) and painful flexor contractures of the fingers of both hands. All these features were considered paraneoplastic but extensive and repeated investigations revealed no underlying malignancy. Oral steroids and orbital radiotherapy were ineffective. The diagnosis was confirmed by trans-scleral biopsy of the right choroid. Rapidly progressive cataracts were treated by phacoemulsification. Severe exudative retinal detachment … Show more

“…Though regression has been reported after removal of the underlying cause, 3 in our case, only some of the lesions showed a decrease in size. Duong et al 4 reported the longest survivor of this disorder (4102 months), but their patient underwent bilateral enucleation; our report shows a case of BDUMP with survival of over 4 years and a good visual acuity.…”

“…1 Since then, it has been described as a paraneoplastic syndrome associated with distal cancers. [1][2][3][4][5][6][7][8] The uveal melanocytic proliferation is a sine qua non of this entity; and in the past, progressive vision loss has been the rule. Along with others, we have recently shown that plasma exchange improves the prognosis.…”

“…[1][2][3][4][5][6][7][8] It is associated with thickening of the uvea because of hyperplastic proliferation of benign uveal melanocytes. [1][2][3][4][5][6][7][8] Recent data show that it can be ameliorated by plasma exchange, suggesting that there is a proliferation-inducing factor in the plasma. 7,8 A recent study has shown that plasma from patients with BDUMP contains an IgG that causes cultured melanocytes to proliferate and elongate, which we have termed cultured melanocyte elongation and proliferation factor (CMEP factor).…”

“…It was previously noted that palmar fasciitis associated with cancer progresses much more rapidly than in Dupuytren's disease (16). In some cases, the resection of the underlying cancer may stop the progression of the symptoms and relieve the symptoms (8,18,19). Only rarely do glucocorticoids produce a favorable effect (18,(20)(21)(22).…”
