The quality of oral health care might be evaluated based on Donabedian's structure-process-outcome model. This study assessed the association between the oral health public services structure and work process of oral health teams (OHT) and performance indicators (access and problem-solving capacity) in Brazil. Secondary data from a national program obtained through interviews and by observation in 2013/2014 were analyzed. The performance indicators were Coverage of First Scheduled Dental Appointment (FDA) (< or � the mean) and Ratio between Completed Treatments and First Scheduled Dental Appointments (CT/FDA) (< 1 or � 1). The structure was assessed by the sum of available instruments, equipment, and supplies. Latent class analyses were used to identify similar groups (consolidated, developing, and incipient) of OHT according to the work process (planning of actions, health promotion and intersectoral actions, and integral health care). Each OHT was also described regarding the number of the health team in which the OHT operates, whether the primary care unit receives students/teaches, frequency of care provided outside of OHT coverage, and participation in telehealth. Multiple logistic regression models were adjusted for each performance indicator. A total of 16189 (99,8%) and 16192 (99,9%) OHTs located in 4344 (78,0%) municipalities had complete data on the work process and structure. 91.92% of OHTs presenting CT/FDA � 1 and 37.05% presenting FDA � the mean. Consolidated planning of actions and better structural conditions were associated with better performance. A higher frequency of CT/FDA � 1 was observed among OHTs with consolidated integral health care and those that performed telehealth. OHTs that served individuals outside of OHT coverage daily and that worked with two to nine Health Teams presented a higher frequency of FDA � the mean. OHTs with better structural and work process conditions had better performance.

Introduction: Epidemiological surveys revealed that Brazil has a high burden of oral diseases. However, no prior study has reported estimates of untreated dental caries, periodontitis, and edentulism over a three-decade period. The objective of this study is to report the trends of prevalence, incidence, and years-lived with disability (YLDs) due to untreated dental caries in primary and permanent teeth, periodontitis, and edentulism in Brazil between 1990 and 2019. Methods: Estimates of prevalence, incidence, and YLDs due to dental caries in primary and permanent teeth, periodontitis, and edentulism were produced for Brazil, by sex and age, between 1990 and 2019, using Dismod-MR 2.1, as part of the Global Burden of Disease Study 2019 (GBD 2019). Trends of oral disorders were analyzed using generalized linear regression models applying the Prais-Winsten method. Results: Almost 100 million Brazilians presented at least one oral disorder in 2019, which was equivalent to a prevalence of 45.3%. All oral diseases combined ranked eighth among all causes of disability, causing more than 970,000 YLDs. Untreated dental caries in primary teeth were estimated to affect 13.5 million children, and untreated dental caries in permanent teeth affected more than 52 million people. Periodontitis affected 29.5 million people, and edentulism affected almost 22 million. The generalized linear regression models revealed a trend of stability of oral disorders between 1990 and 2019. Conclusions: The burden of oral diseases in Brazil is extremely high. Oral disorders, edentulism in particular, caused disability at levels that are comparable to other important chronic diseases.

Resumo Introdução O conhecimento dos fatores relacionados à perda dentária contribui para o planejamento das ações preventivas realizadas pelas Equipes de Saúde Bucal (ESBs). Objetivo Avaliar o indicador de proporção de exodontia nos municípios do estado de São Paulo e a sua relação com os indicadores socioeconômicos e cobertura das ESBs. Método Estudo ecológico com dados secundários dos 645 municípios do estado de São Paulo do ano de 2012. A variável dependente foi o percentual de exodontia mensurado pelo indicador de proporção de exodontia em relação aos procedimentos individuais realizados pelas ESBs, disponibilizado pelo Sistema de Informação da Atenção Básica. Foram considerados como variáveis independentes a cobertura populacional estimada pelas ESBs, o Produto Interno Bruto (PIB) dos municípios per capita, o Índice de Desenvolvimento Humano (IDH) e o Índice Paulista de Responsabilidade Social (IPRS). Foram aplicados modelos lineares generalizados, avaliados pelo p-valor do teste de Wald, AICC e grau de liberdade. Resultados Apresentaram maior proporção de extrações dentárias as cidades do estado de São Paulo com menor cobertura de ESBs (p < 0,0001), com menor valor de IDH (p < 0,0001) e com maior valor de IPRS (p = 0,0018). Conclusão A baixa cobertura de ESBs e as desigualdades socioeconômicas contribuem para aumentar as extrações dentárias no estado de São Paulo.
