Background: Suicide by poisoning is the act of intentionally killing oneself by using various chemical substances and it is the most common and widely used method in developing countries like Bangladesh. Objective: The objectives of this study were to find out the socio-demographic characteristics and possible reasons among the chemical suicidal victims. Materials and method: This was a descriptive type of cross-sectional study, held in the Morgue of department of Forensic Medicine of Dhaka Medical College (DMC), Dhaka, Bangladesh, from July 2016 to June 2018. Victims were selected purposively according to the availability in the morgue of Dhaka MedicalCollege. Data were collected from the relatives of the victims and the verbal consent of the doctors who performed autopsy at DMC morgue. Results: A total of 50 victims of suicide using chemical substances as revealed by autopsy findings from the DMC morgue were enlisted in this study. Victims of suicide not using chemicals were excluded from this study. Half (50.0%) of the suicidal deaths by poisoning cases were aged 20 or below. Majority (66.0%) of the victims was male. Organophosphorus compound was found in 58.0% cases, followed by diazepam (8.0%) and barbiturates (4.0%), after receiving chemical analysis report. Conclusion: Pesticides are the most commonly used suicidal agent particularly in low and middle-income countries like ours. So, social awareness regarding this issue is very much needed. The descriptive observational study was conducted over six months among inpatients of the orthopaedics department. The aim is to observe the prescribing pattern of drugs, to find out the percentage of analgesics given and most commonly prescribed analgesic, to compare the prescribed drugs whether they are in NLEM, WHO list and calculate DDD/100 Bed-days and to analyze the drugs for WHO prescribing drugs. A total of 250 patients were included in the study. Out of this, 168 were male, and 82 were female. Most of the patients were in the age group of 31-40[45 in number with 18%]. The most common condition was found to fracture [113 in number with 95.2%]. Commonly prescribed drugs were analgesics 447 with 26.76%. Among all the NSAID's, PCM was most commonly prescribed analgesic with 34.4%. The highest no, of drugs was found to be 4drugs/prescription with 24%. Monthly one analgesic was prescribed per prescription with 38.8%. Utilization of analgesics in term of DDD/100 Bed-days was 55.26; Drugs will be evaluated per prescription as per prescribing indicators of WHO was done, the average no. of drugs per prescription was found to be 0.45, percentage of drugs prescribed by generic name was found to be 13.3%, percentage of encounters with antibiotics prescribed were found to be 94.4%, In ratio percentage of drugs prescribed from the national list of essential medicine was found to be 92.6%. This study would help to facilitate better health care delivery.

Patterns of prescription and adverse drug reaction profile of Non- Steroidal Anti-Inflammatory Drugs at orthopedic out-patients department

Non-steroidal Anti-inflammatory Drugs (NSAIDs) are the most prescribed drugs all over the world. These are used in the treatment of pain and inflammation. Systematic evaluation of prescription patterns and monitoring of adverse drug reactions is required to increase the therapeutic benefit and decrease the adverse effects of these drugs. An observational, cross-sectional study was conducted for 6 months from September 2021 to February 2022 in 300 patients prescribed least one NSAID to assess the prescription patterns and adverse drug reaction profile (ADR) of NSAIDs prescribed in the orthopedic outpatient department. Among enrolled patients 52% were female and 48% were male. The most common age group was 20-39. The average number of drugs per prescription was 2.89. A total of 868 drugs were prescribed, out of which 402 were NSAIDs (46.31%). Naproxen was the most prescribed agent (45.02%), followed by Diclofenac (17.17%). ADR was reported in 12% of patients. Most of the ADRs were due to Naproxen (72.18%) followed by Ibuprofen (16.66%). The gastrointestinal system was involved in maximum patients and the most common ADR was abdominal pain. Most of the drugs were prescribed by brand name 95.18%. Naproxen was the most prescribed NSAID and responsible for most ADRs. There was a higher prevalence of irrational prescribing, polypharmacy, and underreporting of ADR. A strategy must be developed and implemented for prescribing and rational use of NSAIDs and monitoring their harmful effects.
