Preparation for pyeloplasty for ureteropelvic junction obstruction using a patient-specific laparoscopic simulator: a case report

Abstract: IntroductionTraining systems for laparoscopic surgery are useful for basic training but are not suitable for specific training corresponding to the condition of a given patient. We, therefore, have developed an unusual training system: a patient-specific simulator for laparoscopic surgery. When specific data of each individual patient are entered, this system helps surgeons perform a “rehearsal” operation. We applied this technique in laparoscopic surgery by using volume data obtained by multislice computed to… Show more

“…However, they do not provide surgeons with the necessary experience to respond to specific conditions in individual patients. Therefore, we have developed a unique training system, called a PSVR type simulator, for laparoscopic surgery [ 12 – 15 ]. Using data specific to each individual patient, this system facilitates “rehearsal” operations for surgeons.…”

“…In case 1, in which laparoscopic left pyeloplasty was carried out for ureteropelvic junction obstruction, the operative field contained four renal arteries, two renal veins and a gonadal vein that bifurcated in the middle. These anatomical relationships were too complex to understand by viewing CT images alone, whereas the simulator was capable of accurately depicting it . The other case was case 4, in which retroperitoneoscopic right nephroureterectomy was carried out in a patient with situs inversus totalis.…”

“…Urological procedures performed by laparoscopy require skills other than open surgery and learning can increase surgical time and patient morbidity 1 . Because of the long learning curve, residents should acquire laparoscopic skills before performing procedures in the surgical center 2 .…”
