Preoperative Prediction of Central Lymph Node Metastasis in Thyroid Papillary Microcarcinoma Using Clinicopathologic and Sonographic Features

Abstract: Central lymph node metastasis in patients with PTMC was significantly associated with various clinicopathological factors, including larger tumor size (>5 mm), bilaterality, extracapsular invasion, and lateral LNM. When these features are detected on preoperative US, selective central compartment dissection may be helpful in patients diagnosed with PTMC.

“…Few studies have reported the associations between US characteristics and CLNM. In the present study, no significance was identified for these associations, which is consistent with a previous study by Kim et al (24). A number of previous studies have reported that age is not associated with CLNM in patients with PTMC (24,25).…”

“…In the present study, no significance was identified for these associations, which is consistent with a previous study by Kim et al (24). A number of previous studies have reported that age is not associated with CLNM in patients with PTMC (24,25). However, Wang et al (26) demonstrated that there is a significant difference between patients <45 years old and ≥45 years old in cN0 PTMC.…”

“…Kim et al (2013) also demonstrated that multifocaltiy is an independent predictive factor for central LNM. In our study, 32.6% of PTMC patients was found multifocality, and 45.2% of them had central LNM, which , in univariate analysis, was significantly higher than unifocal (p=0.002).…”

“…Several studies reported that the tumor recurrence rate increases in multifocal tumors (24,25). In one study, the risk of CLNM is higher in bilateral tumor because of the tendency of large tumor size and more frequent extrathyroidal invasions (26,27). However, according to Zhao et al (28), not only multifocality but both tumor size and multifocality are considered to be related to CLNM.…”
