Preoperative evaluation of living kidney donors using multirow detector computed tomography: comparison with digital subtraction angiography and intraoperative findings

Abstract: Summary To assess the accuracy of multirow detector computed tomography (MDCT) for the evaluation of renal anatomy for preoperative donor assessment in living related kidney transplantation. MDCT‐scans (4‐ and 16‐slice‐CT) of 51 consecutive living kidney donors (age, 51.6 ± 9.7 years; range, 28–68 years) were analysed by three blinded observers and compared with digital subtraction angiography (DSA) and surgery. Contrast‐enhanced MDCT was performed with 1 mm slice thickness reconstruction interval during arter… Show more

“…ERA and ED are of particular importance for the decision on the site of nephrectomy. 12 The presence of renal artery variations in the donor kidney substantially increases the risk of complications, given the technical difficulties and longer anastomosis time. 6,13,14 The successful implantation of a renal stent also requires precise knowledge of the origin of the renal arteries from the aorta.…”

“…3,4,7 The use of computed tomographic angiography (CTA) to assess the quality of donor kidneys and the renal vasculature is part of the donor workup. 3,5,8 There are inherent risks associated with the medical and radiological evaluation itself, including incidental pathological findings and the emotional impact of being turned down for donation. 7 One review of 1600 potential donors demonstrated that 49% were excluded for medical and ethical reasons.…”

“…A comprehensive anatomic evaluation of potential renal donors is crucial when harvesting is performed using the laparoscopic technique. Multiphase MDCT is an accurate and useful technique for providing a comprehensive preoperative evaluation of renal donors [19,22,23].…”

“…MPR and CPR images are particularly useful for correct evaluation of the arterial luminal diameter [21]. VR images can be used for the overall display of the abdominal vasculature and can provide an insight for the interpreter and referring physicians [22,10]. Finally, the axial source images should always be reviewed for possible presence of an accompanying non-vascular pathology.…”
