Preoperative Endocrine Therapy for Older Women with Breast Cancer: Renewed Interest in an Old Idea

Abstract: Preoperative endocrine therapy is a logical approach for older patients with ER+ disease as a well-tolerated means to increase the rate of breast-conserving surgery. Several clinical trials comparing tamoxifen with selective aromatase inhibitors in the preoperative setting have been conducted, and the results are expected soon. These studies will determine if a large multicenter national trial of preoperative endocrine therapy should be conducted.

“…This may have led to an underestimation of tumor responses. A review of the literature suggests that the initial response rates to preoperative endocrine therapy exceed 50-60% for patients with receptor-positive disease [12]. 79 of 134 cases (59%) in the chemo-/radiotherapy and 29/64 cases (45%) in the chemotherapy group received preoperative endocrine treatment which implies that the difference in complete remission rates in both groups cannot be attributed to the additional endocrine therapy.…”

“…[1][2][3][4] Such long-term therapy was used in those old, frail, and infirm patients who could not tolerate surgery. Since that time, the concept of using a medical treatment before surgery has developed.…”

“…In these cases, TAM is therefore administered as a secondline therapy. There is also increasing amounts of data supporting the use of aromatase inhibitors in the preoperative setting, which may lead to increased opportunities for breast-conserving local therapy, as well as suitable treatment alternatives for those who are not candidates for surgery or chemotherapy (Dixon, 2004;Dixon and Miller, 2003;Ellis, 2000).…”
