Preoperative cognitive function predicts survival in patients with resectable pancreatic ductal adenocarcinoma

Abstract: Presurgery cognitive function is a significant independent predictor of survival in patients with resectable PDAC. Thus, presurgery patient reported outcomes may provide as strong prognostic information as clinicopathological factors.

“…After removal of 690 duplicates, a further 2588 articles were excluded after title and abstract screening because they did not meet the prespecified inclusion criteria. The full texts of the remaining 468 studies were evaluated in more detail, and 89 studies were found to meet the eligibility criteria for this systematic review. Among them, 22 studies containing information on clinical factors relevant to recurrence sites were selected for the meta‐analysis.…”

“…The Cochrane Collaboration tool for assessing risk of bias showed that one study had no domain with a high risk of bias and three studies had one of seven domains with a high risk of bias ( Table S3 , supporting information). The overall risk of bias in non‐randomized studies, according to ROBINS‐I, was moderate in 48 studies and serious in 37 studies ( Table S4 , supporting information).…”

“…The notion that pre-treatment cancer patients experience neurocognitive decline more readily than age-matched controls is gaining attention. Furthermore, recent clinical studies demonstrate that cognitive decline prior to cancer treatment is a significant independent predictor of survival [21,22]. Taken together, the clinical studies to date suggest that peripheral tumors and blood cancers affect several domains of cognitive function and modulate white matter neuroanatomy.…”

“…However, less was known about the prognostic utility of CRCI until recently. Cognitive impairment (deficits in working memory) prior to cancer treatment is an important predictor of survival in hematologic cancers [21] and pancreatic cancer [22]. Furthermore, CRCI is not limited to older patients, as it is also observed in younger patients with Hodgkin lymphoma [23].…”

“…However, when patients are diagnosed with pancreatic cancer, more than half are at an unresectable stage and are given chemotherapy, radiation, palliative surgery or supportive care ( 8 , 9 ). Even if patients undergo curative resection, their 5-year survival rates are approximately 15–20%, with a median survival of 16 to 23 months ( 10 13 ).…”
