Prenatal diagnosis of placenta previa/accreta with color Doppler ultrasound

Abstract: A case of placenta previa/accreta was diagnosed at 23 weeks' gestation using color Doppler ultrasound. Imaging showed an unusually intense blood flow within the placental sonolucent spaces and the hypervascularization patterns within the placental and the non-placental tissues. Highly pulsatile venous flow patterns were detected within the placental blood lakes and the subplacental venous complex. A strong association between the lacunar flow pattern and the abnormally adherent placentation was confirmed by th… Show more

“…Kastner et al found that the rates of placenta creta were 35, 56, 75, and 100% after zero, one, two, and more than two CSs, respectively (15). Chou et al identified 14 patients with a histologically proven placenta accreta, of which seven (50%) had had a previous CS (16). In our smaller study, the incidence of placenta creta was 20, 10, and 70%, after zero, one, and two or more CSs, respectively.…”

“…In placenta creta cases, remodeling of the larger radial and arcuate arteries is seen (3). In these arteries, the loss of muscular and elastic tissue render them unresponsive to vasospasm because of their larger diameter and greater blood flow compared to the smaller diameter spiral arteries (3,9,16). Thus, when removal of the adherent placenta is attempted, heavy haemorrhage occurs.…”

“…The depiction of turbulent blood flow in placental lacunae and the presence of low-resistance arterial blood flow in the serosa-bladder interface on color Doppler imaging are highly predictive of accreta. 30 If the ultrasound findings are not considered definitive or the placenta is located on the posterior wall or to assess possible bladder involvement, MRI can be performed using gadolinium contrast intravenously. Magnetic resonance imaging findings suggestive of placenta accreta include placental heterogeneity; mass effect of the placenta into the underlying bladder or extending laterally or posteriorly beyond the normal uterine contour; obliteration of the myometrial zone visible on initial uptake of gadolinium and a beading nodu larity within the placenta.…”

“…8 However, in a small series of 6 cases, Levine et al 5 found that color Doppler imaging added no appreciable information in diagnosing placenta accreta. False-positive diagnoses may occur with these criteria because of technical factors and the presence of bladder varices; therefore, it is important that the examination is made with the proper resolution and focus using both transabdominal and transvaginal transducers.…”
