Delayed ejaculation (DE) is one of the most difficult disorders to diagnose from male sexual dysfunctions. It is characterized by a significant ejaculation delay or even an inability to achieve ejaculation (AE). It is a disorder that affects men, but it can also have a secondary effect on their wives/partners and possibly their ability to fertilize. The 5th edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM) describes this sexual disorder as ejaculation which is delayed or absent in at least 75% of sexual intercourse during at least 6 months of partner sexual activity without the man's desire (Abdel-Hamid & Ali, 2018). DE itself also brings anxiety and fear to men (Rowland et al., 2010). Delayed ejaculation can be defined as the time it takes a man to reach ejaculation during vaginal penetration. This time is called IELT (intravaginal ejaculation latency time). The 3rd International Sexual Medicine Consultation defined IELT as a period of 20-25 min during which sexual activity lasts. This time was chosen based on the average IELT obtained in studies in the general male population. Some studies report that the normal range for IELT in men is 4-10 min (Perelman, 2016). The exact pathogenesis of DE/AE is not yet known. There are studies that suggest that this sexual disorder may be caused by a congenital disorder, a genetic disease, surgical complications, hormonal disorders, prostate or seminal vesicles inflammations or diseases, or pelvic nerve disorders (Corona et al., 2011). Our study looked at one of the possible causes of DE/AE, and that is hyperprolactinemia caused by antidepressant treatment in depressive disorders. We evaluated the interrelationships between hyperprolactinemia, depressive disorder, and delayed ejaculation or anejaculation. Lithium has proven its efficacy in treating bipolar disorder. Severe side effects caused by lithium, including renal and endocrine outcomes, have already been amply documented. The impact of lithium on sexual function, however, is less well known. A 33-year-old man, with no past medical history, diagnosed with bipolar disorder, developed premature ejaculation after short-term use of lithium. The dose of lithium was reduced, leading to a rapid clinical resolution. Retrospectively, lithium-induced premature ejaculation was deemed the most likely diagnosis. Premature ejaculation is a rare side effect of lithium. Changing the time of medication administration and lowering dose could be considered as alternatives. Given lithium’s pharmacological profile, it is likely that the pathophysiologic mechanism behind premature ejaculation is altered levels or altered serotonin receptor sensitivity in the ejaculatory modulating centers of the central nervous system. Given the reluctance to spontaneously report sexual adverse effects, clinicians should be aware of this possible side effect.

The Effects of Steady Freddy, a Lidocaine-Based Pump Spray for the Treatment of Premature Ejaculation (PE)

Premature ejaculation (PE) is reported to be the most common sexual dysfunction in men and is defined as the inability to control or delay ejaculation. Steady Freddy is a lidocaine-based ejaculation-delaying spray. This article examines the effects of Steady Freddy on the sexual experience of men that have self-reported to suffer from PE. Under the conditions of a randomized single-blind placebo-controlled clinical trial, 150 participants received either placebo or treatment for the duration of 12 weeks and completed an internet-based questionnaire for the quality of their sexual experience. Prior to product usage, participant average latency time was <1 min, 70% claimed to be very sexually dissatisfied, and 67% claimed to be very dissatisfied with ejaculation control. Upon product usage, sexual experience was significantly improved ( p<.01). Participant average latency time increased to >2 min, 80% claimed to be sexually satisfied, and 70% claimed to be satisfied with ejaculation control. These effects were not present in the placebo group throughout the trial. These findings provide evidence for the effectiveness of Steady Freddy in significantly improving the quality of sexual experience and suggest that Steady Freddy can assist with PE.
