Pregnancy in Sickle Cell Disease in the Al Hassa Region of Saudi Arabia

Abstract: Pregnancy in sickle cell disease (SCD) patients is associated with increased risk to both mother and fetus. 1 The risk varies in different geographical areas. Very high maternal mortality rates of 11.5% have been reported from West Africa and from black American groups.2 These high mortality rates have led to suggestions by clinicians that pregnancy in SCD patients is contraindicated and have advised therapeutic abortion and sterilization. “…This is the largest survey on the effects of SCD in pregnancy carried out in the Gulf States [9–12]. Not only maternal mortality, but the incidence of cesarean sections, preterm labor, and pregnancy morbidity were higher among women with SCD than in the control group [13–15]. The complex of pulmonary and renal infections, preterm labor, and lower birth weight are “defining” characteristics of this condition during pregnancy and delivery.…”

“…The high HbF levels protect against several clinical features associated with SCD, but the association between HbF levels and the severity of the disease process is complex. [224] As in other studies, removal of the appendix from all patients was made easily through an incision over McBurney's Point. [25] Laparoscopic appendectomy has gradually gained universal agreement for the treatment of acute appendicitis; however, the controversy still exists over the safety and feasibility of a laparoscopic appendectomy during pregnancy.…”

“…[1] Pregnancies in SCD patients present a clinical challenge as maternal mortality is 1-2% and perinatal mortality is 5-6%. [23] Acute appendicitis is the most common extrauterine surgical emergency seen in pregnancy, and appendectomy is the most common nonobstetric procedure being performed on pregnant patients. [4–7]…”
