Ovarian cryopreservation is a promising technique to preserve fertility in women with Hodgkin lymphoma (HL) treated with chemotherapy. Thus, the aim of this study was to examine harvested ovarian tissue for subclinical involvement by HL by morphology/ immunohistochemistry, and to define patient and treatment factors predictive of oocyte yield. This was a retrospective analysis of 26 ovarian tissue samples harvested for cryopreservation from women with HL. Histology, immunohistochemistry and follicle density (number mm À3 ) was examined. Disease status and preharvest chemotherapy details were obtained on 24 patients. The median age was 22 years (range 13 -29). Seven of 24 patients had infradiaphragmatic disease at time of harvest. Nine of 20 patients had received chemotherapy preharvest (ABVD (Adriamycin s , Bleomycin, Vinblastine and Dacarbazine) ¼ 7, other regimens ¼ 2). The seven receiving ABVD showed no difference in follicle density compared to patients not receiving treatment (n ¼ 14); (median ¼ 1555 vs 1620 mm 3 P ¼ 0.97). Follicle density measurement showed no correlation with patient age (R 2 ¼ 0.0001, P ¼ 0.99). There was no evidence of HL involvement in the 26 samples examined (95% CI ¼ 0 -11%). In conclusion, subclinical involvement of HL has not been identified in ovarian tissue, even when patients have infradiaphragmatic disease. Furthermore, the quality of tissue harvested does not appear to be adversely affected by patient's age or prior ABVD chemotherapy.

RESUMOCom a evolução dos recursos terapêuticos e o aumento das taxas de sobrevida dos pacientes oncológicos, as repercussões tardias destas terapias, que antes eram infreqüentes, hoje assumem um papel importante quando se fala em qualidade de vida. Dentre estas complicações tardias está a perda da função ovariana. Segundo as recomendações da Sociedade Americana de Oncologia Clínica (American Society of Clinical Oncology) recentemente publicadas, os métodos comprovadamente efi cazes para preservação da fertilidade feminina disponíveis hoje são: o congelamento de embriões, a cirurgia ginecológica conservadora e a ooforopexia para os casos de radioterapia localizada. Todas as demais técnicas existentes, tais como a supressão ovariana medicamentosa e a criopreservação de tecido ovariano e de oócitos, embora apresentem resultados promissores, ainda são consideradas experimentais. A escolha da melhor técnica para preservação de fertilidade aplicável em cada caso vai depender da idade da paciente, do tipo de tratamento, da existência ou não de parceiro com quem deseje constituir prole, do tempo disponível até o início da quimioterapia e do potencial do câncer em produzir metástase ovariana. Neste artigo as técnicas disponíveis e experimentais para a preservação da fertilidade são revisadas e discutidas. PALAVRAS-CHAVE:Falência ovariana prematura; Criopreservação; Quimioterapia; Radioterapia; Infertilidade ABSTRACTAs therapeutic approaches for oncologic diseases are being improved and an increase in the survival rates are being achieved, long-term complications of these therapies, initially infrequent, assume these days an important place when considering life quality. Among the long term repercussions appears the premature ovarian failure. According to the recommendations of the American Society of Clinical Oncology recently published, the only procedures available nowadays considered to be effective for female fertility preservation are: embryo cryopreservation, conservative gynecological surgery and oophoropexy in cases of local radiotherapy. All the other proposed techniques, surch as: ovarian suppression and oocyte and ovarian tissue cryopreservation, although present promising results, are still considered as experimental options. The best choice for fertility preservation in each specifi c case depends on patient's age, type of treatment, existence of a partner, time available until chemo-or radiotherapy beginning, and the ovarian metastatic potential of the tumor. In the present manuscript, the available and experimental techniques for fertility preservation are revised and discussed. KEYWORDS:
