Predictors of Youth Diabetes Care Behaviors and Metabolic Control: A Structural Equation Modeling Approach

Abstract: Continued parental supervision of adolescents, along with monitoring diabetes knowledge and efficacy, may help optimize transfer of diabetes care from parents to youths. Behavior problems warrant immediate attention because of their direct and adverse relation to metabolic control.

“…Stress is a process in which environmental demands tax or exceed one's adaptive capacity, resulting in physiological changes that may place the individual at risk for disease over time (1)(2)(3)(4)(5)(6)(7). Chronic stress may manifest itself in various ways including increased anxiety, depressive symptoms, hostility and poor sleep quality, all which independently predict essential hypertension (EH), cardiovascular disease (CVD), metabolic syndrome and type 2 diabetes (1)(2)(3)(4)(5)(6)8).…”

Stress exposure also increases unhealthy coping behaviors such as comfort eating, smoking, excessive alcohol intake, pharmaceutical abuse and increased sedentary activity.…”

“…Memory problems are linked to poorer performance on a wide range of cognitive measures, and consequently youth may never fully attain prediabetes intellectual or vocational potential (Ryan, 1999). Further, in adolescence memory relates to successful completion of disease care requirements (Gillespie, Kulkarni, & Daly, 1998;Holmes et al, 2006;Soutor et al, 2004). Measures of memory also can be a sensitive indicator of subtle decrements in cognitive function and are among the best predictors of clinically significant cognitive deterioration (Diabetes Complications and Clinical Trial [DCCT] Research Group, 1994).…”

“…In case of a negative and critical relation with the parents and lack of support, when nobody assumes the responsibility for disease management or when tasks related to the regime are not shared, metabolic control gets worse (23) . Stress-loaded environments, inside or beyond the family context, can entail behavioral problems that, consequently, interfere in self-care, affecting, among other issues, the children's diet (18) , who can be more anxious and depressed (24) . Health professionals should pay attention to the constant need for family assessment and follow-up, as children who are not closely supervised by their parents require special attention, mainly regarding the frequency of meals (24) .…”

Health professionals should pay attention to the constant need for family assessment and follow-up, as children who are not closely supervised by their parents require special attention, mainly regarding the frequency of meals (24) . Likewise, in daily experiences with the disease, single-parent families can present difficulties to support these children, so that relatives and friends need to be mobilized to share care (22) .…”
