Predictors of mortality in patients with severe sepsis or septic shock in the ICU of a public teaching hospital

Abstract: Background: We hypothesized that one single episode of acute kidney injury (AKI) reduces long-term survival compared with no acute kidney injury (No AKI) following recovery from critical illness. Materials and methods: A prospective cohort of 2,010 patients admitted to the ICU between 2000 and 2009 at a provincial referral hospital was followed to determine whether AKI influences long-term survival. Results: Of the 1,844 eligible patients, 18.4% had AKI stage 1, 12.1% had stage 2, 26.5% had stage 3, and 43.0% … Show more

“…These findings are similar to those of de Oliveira et al . [ 31 ], where not the PCT level but the SOFA score value was highly associated with mortality in ICU patients with severe sepsis and septic shock. In our study in severe sepsis and septic shock subgroups the values of PCT and SOFA score were significantly different on the 1 st day of therapy, similarly to Lavrentieva's study [ 32 ].…”

“…Although other studies are required to evaluate the accuracy of these data in different populations, a recent meta-analysis found that IL-10 polymorphisms are associated with sepsis susceptibility in Caucasian and Asian populations ( Pan et al 2015 ). Furthermore, in addition to the risk of septic shock in this study possibly due to increased IL-10 plasma concentrations ( Wang et al 2011 ), it is now well-known that early sepsis diagnosis and correct clinical support compliance play a critical role in mortality ( de Oliveira et al 2013 , van Zanten et al 2014 ). In this context, we speculate that IL-10-induced immunosuppression may have a more important impact on the restriction of infection than on sepsis outcome.…”
