Predictive value of carcinoembryonic antigen and carbohydrate antigen 19-9 related to downstaging to stage 0–I after neoadjuvant chemoradiotherapy in locally advanced rectal cancer

Abstract: ObjectiveTo explore the value of carcinoembryonic antigen (CEA) and carbohydrate antigen 19-9 (CA19-9) in predicting downstaging to stage 0–I cancer after neoadjuvant chemoradiotherapy (nCRT) in locally advanced rectal cancer.Materials and methodsWe respectively investigated pretreatment CEA, pretreatment CA19-9, posttreatment CEA, posttreatment CA19-9, pre–post-CA19-9 ratio, and pre–post-CEA ratio in 674 patients with locally advanced rectal cancer receiving nCRT and determined the patients’ thresholds by usi… Show more

“… 24 The results of previous studies showed that the level of CA19-9 before treatment could predict whether patients could benefit from nCRT. 25 , 26 In this study, 15 of 20 patients who had a positive CA19-9 before nCRT have no down-staging after nCRT, which is consistent with the above conclusion. It also suggests that we should pay more attention to the significance of CA19-9 in the prognosis and monitoring of rectal cancer.…”

“…Multiple scholars reported that CA19-9 was good predictor of response to nCRT and survival after curative surgery of rectal cancer. 10,[17][18][19] This study also showed pretreatment CA19-9 and posttreatment CA199 were prognostic factors for overall survival of LARC patients with nCRT. Some studies demonstrated that neoadjuvant chemoradiotherapy have decreased lymph node yield.…”

“…Capecitabine was taken with the dosage of 825mg/m 2 twice daily. And the other specific protocol of chemotherapy has also been reported previously 10 The capecitabine or 5-FU based neoadjuvant chemotherapy was initiated on the first day of radiotherapy.…”

“…A previous study from our center demonstrated that pretreatment CEA and pretreatment CA199 could reflect responses to CRT therapy because the pCR rate revealed long-term oncological outcomes to some extent [15]. However, we did not previously perform a survival-related analysis due to the follow-up time being too short, so we explored this further in the present research.…”

“…The method of radiotherapy has been described in a prior published paper [15, 16]. Regarding target volumes, we set tumor/tumor bed as the gross tumor volume (GTV) based on examinations.…”
