Prediction of Outcome of Decompression for Carpal Tunnel Syndrome

Abstract: 45 patients (55 hands) with carpal tunnel syndrome treated surgically have been studie Painful nocturnal paraesthesiae occurred in patients with short histories and was a good prognostic feature. Electromyography proved of value in assessing motor lesions and the degree of the process. The first sign of muscle involvement was fibrillation potential. In longer histories an increased amount of polyphasic potentials and reduced voluntary pattern was seen. The reduced pattern is reversible in cases of short durati… Show more

“…Another reports that patients with middle‐grade NCS abnormalities for CTS, rather than very severe or no abnormalities, have the best outcomes 36 . Severe degeneration on needle EMG has been associated with worse outcome 43 . Comparable to the UNE literature, these reports have clinical heterogeneity as surgical techniques have progressed and definitions for diagnosing CTS have matured through the years.…”

“…36 Severe degeneration on needle EMG has been associated with worse outcome. 43 Comparable to the UNE literature, these reports have clinical heterogeneity as surgical techniques have progressed and definitions for diagnosing CTS have matured through the years. However, one major difference between the CTS and the UNE literature is the use of a severity classification systems for CTS 44,45 and the lack of clinical uniformity in severity classification for UNE.…”

“…Debate continues regarding how symptom duration affects surgical outcomes. Some studies have suggested that a longer duration is associated with worse outcomes (i.e., less improvement in symptoms after surgery) [5,7,8,34]. Long-term exposure to entrapment can cause severe nerve damage, including irreversible axonal damage, leading to a poor prognosis.…”

“…Although patients with thenar atrophy may still have good recovery after CTR, worse outcomes are seen in patients with thenar atrophy compared with patients without preoperative thenar atrophy. [63][64][65][66][67][68] Not every study shows that thenar atrophy is associated with worse outcome after CTR, which may be caused by a lack of power or inaccurate measurement. 69,70 Presence of thenar atrophy can only be assessed subjectively as being present or absent.…”
