Ocular surface squamous neoplasia (OSSN) Corneal and conjunctival squamous lesions are uncommon but important because of their potential for causing ocular and even systemic morbidity and mortality. The clinical presentation of these lesions varies across a wide spectrum and can range from mild to severe dysplasia to full-thickness epithelial dysplasia (carcinoma in situ) and invasive squamous cell carcinoma. Squamous lesions can involve the conjunctiva or the cornea, but more commonly start in the conjunctiva and extend across the limbus to the adjacent cornea. Even though the first case was described in 1860 by von Graefe 1 and has been extensively described after that, the management of these lesions has changed significantly in the past decade.The initial cases of squamous neoplasms described in the literature were cases of squamous cell carcinoma. 2 Subsequently, it has been recognized that both invasive and non-invasive subtypes of squamous neoplasms occur. Purpose To analyze the correlation between the onset of ocular surface squamous neoplasia (OSSN) and viral infections such as human papillomavirus (HPV), as well as ocular surface colonized pathogenic microorganisms. Methods Three patients who were clinically diagnosed with OSSN and underwent ocular surface tumor resection combined with amniotic membrane transplantation from March 1, 2023 to June 1, 2023 were selected as the study group. Specimens were collected during surgery for pathological examination and metagenomic next-generation sequencing (mNGS) of pathogenic microorganisms. Another three patients diagnosed as corneal leukoplakia who were tested negative for viruses by mNGS after penetrating keratoplasty during the same period were selected as the control group. Results Among the 3 OSSN patients, 1 was male and 2 were female, with an average age of 71.3 ± 4.0 (69–76) years. The OSSN in all 3 cases invaded both the cornea and conjunctiva. In this group, mNGS suggested no detection of viral pathogens in the specimens. However, suspected colonized bacteria were detected in the 2 cases of corneal and conjunctival intraepithelial neoplasia (CIN): Propionibacterium acnes, Moraxella osloensis, Pseudomonas fluorescens, Prevotella melaninogenica, and in the 1 case of squamous cell carcinoma of conjunctiva and cornea (SCC): Propionibacterium acnes. Among the 3 cases of corneal leukoplakia in the control group, mNGS indicated no viral pathogens, but detected suspected colonized bacteria of Propionibacterium acnes, Lactobacillus inerta, Staphylococcus hominis, Staphylococcus cohnii, Staphylococcus epidermidis and Escherichia coli. Conclusion The occurrence of OSSN of the patients in this paper was not related to HPV infection. Nevertheless, the increased presence of colonized bacteria of Moraxella osloensis, Pseudomonas fluorescens and Prevotella melaninogenic, as well as the decreased presence of Lactobacillus inerta, Staphylococcus hominis, Staphylococcus cohnii, Staphylococcus epidermidis and Escherichia coli may be the high-risk factors for OSSN.

Jelen összefoglalóban a szemfelszíni laphám-neoplasiáról való ismereteket összegzik a szerzők, beleértve ezen daganattípus epidemiológiáját, etiológiáját, patológiáját, klinikai megjelenését és kezelését. A conjunctivalis intraepithelialis neoplasiával és az invazív laphámrákkal kapcsolatos közleményeket tekintették át a szerzők néhány saját eset bemutatásával, amelyek alapján olyan alapvető írást szeretnének közölni, amely részletesen taglalja és bemutatja jelenlegi ismereteinket a szemfelszíni laphám-neoplasiáról. A szemfelszíni laphám-neoplasia a leggyakoribb szemfelszíni malignus tumor, illetve a choroidealis melanoma malignumot és a lymphomát követően a harmadik leggyakoribb okuláris malignoma. Alacsony malignitási potenciálja ellenére előrehaladott stádiumban jelentős látásromláshoz vezethet, sőt akár a szemgolyó eltávolításának szükségessége mellett távoli metasztázis kialakulásakor az életet is veszélyeztetheti. Gyakori recidívaképződés miatt kezelésében elengedhetetlen az intra-és posztoperatív adjuváns kezelési módok ismerete és alkalmazása, valamint a rendszeres kontrollvizsgálat. A szemfelszíni laphám-neoplasia felismerése és adekvát kezelése elengedhetetlen ezen tumortípus progressziójának elkerülésében és a recidíva kialakulásának megelőzésében. We summarize up-to-date literature on conjunctival intraepithelial neoplasia and invasive squamous cell carcinoma and present some own cases. Ocular surface squamous neoplasia is the most common malignant ocular surface tumor and the third most common ocular malignancy following malignant melanoma and lymphoma. In spite of its low malignant potential, in advanced stages it may reduce visual acuity significantly or even the eye globe has to be removed. In case of metastasis it may also be life-threatening. As local recurrences of ocular surface squamous neoplasias may occur, knowledge of intra-and postoperative adjuvant treatment options are indispensable and regular control examinations are necessary. Identification and adequate treatment of ocular surface squamous neoplasias are necessary in order to avoid its progression and to prevent recurrences.
