Preadmission statin use improves the outcome of less severe sepsis patients - a population-based propensity score matched cohort study

Abstract: In this national cohort study, preadmission statin therapy before sepsis development was associated with a 12% reduction in mortality when compared with patients who never received a statin. There were no consistent beneficial effects of statins in all patient subgroups.

“…Statins, hydroxymethylglutaryl-CoA reductase inhibitors, may have effects on inflammatory diseases such as sepsis and ARDS via its multiple properties, including an anti-inflammatory effect and inhibition of leucocyte adhesion. 35,36 Statins have become a controversial treatment for sepsis and infectious diseases in recent years. [36][37][38][39][40] Due to the poor water solubility and strong fat solubility of statins, NP carrier systems have been designed to enhance drug delivery, 41 and it has been confirmed that SV-NPs have good efficacy towards chronic inflammatory disorders such as atherosclerosis and chronic pulmonary disease.…”

“…Our sensitivity analysis of propensity score–matched cohorts [ 54 ] revealed that statins were associated with a lower pneumonia and TB risks. This result may be explained by the different cohort and statistical methods used in the current research.…”

“…22 Pre-admission statin use has also been found to improve outcome in moderate sepsis in a large retrospective observational study. 23 There is also preliminary evidence showing the beneficial effects of restarting statins postoperatively. 24 Among 300 000 patients who had been taking statins before non-cardiac surgery, 30 day mortality was 40% greater (2.6%) in those patients who failed to restart their statin within 48 h postoperatively, compared with those who quickly resumed or never stopped taking their statin.…”
